Job summary

We are looking to recruit a registered Nurse with experience in delivering the management of diabetes.

Selsey Medical Practice are part of Chichester Association of Medical Practice Primary Care Network.

We are committed to improving the care provided to patients, reducing health inequalities and raising the quality and standards of GP practice. This role will part of the developing model of care which addresses our ambition to deliver person-centred coordinated care and support implementation of the NHS long-term plan.

Main duties of the job

Practice nurses are part of our multi disciplinary team delivering chronic disease management clinics.

The successful candidate will:

  • Provide compassionate, safe and effective care and support to patients.
  • Prioritise and manage their own workload, recognising where elements of care can safely be delegated to other colleagues, carers and family members.
  • Provide and receive sensitive information concerning patients medical conditions

About us

Selsey Medical Practice is part of the Chichester Alliance of Medical Practices (ChAMP) Primary Care Network which consists of 8 GP surgeries covering Chichester, Selsey, Wittering, Tangmere and Southbourne and provides care to over 92,000 patients.

We have 12,500 patients registered at our practice, we have 11 GP's who are supported by a team of nurses and other healthcare professionals and supporting roles.

We are a training practice, with 2 GP trainers.

Job responsibilities

Chronic disease management

To implement and evaluate individual treatment plans for pre-diabetic / diabetic patients that promote health and wellbeing

To be involved in influencing procedures and protocols for the management of chronic diseases.

Maintain an awareness of the developments in the care and treatment of patients in order to be able to carry out duties competently

Infection Control

To maintain a clean, safe environment, ensuring adherence to standards of cleanliness, hygiene and infection control.

Make effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes.

Person Specification

Qualifications

Essential

  • RGN & NMC Registration
  • Qualification in management of diabetes

Desirable

  • Mentor or teaching qualification
  • Post graduate diploma or degree (Chronic Disease Management)
  • Nurse prescriber
  • Family planning qualification

Knowledge and Skills

Essential

  • Requesting pathology tests and processing the results, advising patients accordingly
  • Understanding the importance of evidence-based practice
  • Understand the requirement for PGDs and associated policy
  • Ability to record accurate clinical notes
  • Ability to work within own scope of practice and understanding when to refer to GPs
  • Chaperone procedure
  • Chronic disease management in diabetes

Desirable

  • Broad knowledge of clinical governance
  • Knowledge of public health issues in the local area
  • Awareness of issues within the wider health arena
  • Knowledge of health promotion strategies
