Job summary
We
are looking to recruit a highly motivated enthusiastic band 5 staff nurse to
work in Halton treatment rooms. Applicants must have excellent interpersonal and
communication skills, have a Patient centered approach to care, be highly
motivated and able to work autonomously as well as part of a team.
The treatment room service is for
non housebound patients, providing a
comprehensive range of clinical skills across 7 sites in Halton (Widnes and Runcorn) . Hours are 8.30
4.30. Currently the service is Mon Fri with one clinic site operational Saturday and Sunday. Weekly
evening clinics sessions are planned for the future to meet patient needs.
The Treatment room service is
committed to supporting personal development. We offer a supportive 4 week supernumerary
period for all new starters. We have an in house training programme for
Mentorship and a 4 day Comprehensive leg Ulcer course, to support all new
starters within Bridgewater. We also encourage staff to complete a credited
nurse prescribing module.
Candidates must be able to travel independently accross the trust.
Main duties of the job
You will work autonomously in a clinic setting delivering:
Medication administration /Medicines
management / Injections
Health Promotion
Holistic assessments
planning and evaluation of patients care needs.
Patient Centered Care
Complex wound management/Doppler
assessments/Compression Bandaging
Ear Syringing
Routine re catheterisations for ambulatory patients
Mentoring, Teaching and Assessing
students.
In addition the post holder will actively support the Treatment Room
Service by undertaking responsibility for delegated patients within a clinical
setting. Liaising with the Treatment
Room team leader, District Nurse Coordinator and District Nurses will ensure information sharing in relation to patient
care / changes to care plans and the continuity of care and reviewing of
complex care packages.

Duties and Responsibilities:
Clinical:
To promote the health and
wellbeing within the family unit and community as a whole.
Perform comprehensive assessment
of patient nursing needs as the named nurse.
Plan, assess and evaluate programmes of care
according to changing health needs and if needed under the direction of the Tissue Viability
specialist Nurse.
To support the team leader in the co-ordination of complex
and continuing care.
Maintain accurate records in accordance with NMC.
Attend a multi-disciplinary case conference as the named
nurse if appropriate.
To communicate and collaborate
with other members of the PHCT, voluntary and statutory organisations to
support individual patient care.
To establish and maintain
effective communication with patients, relatives and carers in complex and
potentially challenging situations.
To competently perform clinical
skills related to the post.
To be professionally responsible
for a delegated caseload under the supervision of the Team Leader.
To assess health needs at
individual, practice and community level.
To work without direct supervision.
Managerial:
To access support from the Treatment room team leader and
Coordinator within the group for the day to day workload.

the team.

Act as a mentor for pre-registration

training and other training that supports the role,

Undertake training that is specific
to the post including non medical nurse prescribing course.
Teaching of patients / carers and
other professionals to support patients to maintain their optimum level of
independence.
Participate in the PDP process.
To adhere to all Trust Policies.
Report sickness and other absences
to the identified administrator.
To provide accurate, timely
information to the identified administrator regarding annual leave, expenses
and study leave.
