Job summary

The Armada Family Practice are looking for a enthusiastic advanced practitioner to join our friendly and supportive team.

Main duties of the job

Main Objectives of Post

  • To provide holistic health care for patients including those with undifferentiated and undiagnosed problems using advanced clinical skills.
  • To assist the workload of the GPs by dealing with requests for advice, appointments and visits, and carrying out consultations with patients as required within the scope of the post, prescribing as necessary within guidelines for non-clinical prescribing.

Our clinical team comprises of GP Partners, Salaried GPs, Advanced N Practitioners, Paramedic Practitioners, Practice Community Nurses, Practice Pharmacists, MSK practitioners, Mental health nurse, Health and Wellbeing coach, Practice nurses, HCAs, and a large administration team.

All our appointments are booked by our experienced care navigation team who ensure that patients are booked in with the right clinicians at the right time. They use a navigation tool called Sentiers which has been adapted to meet our needs and appointment system.

Job description

Job responsibilities

1. In conjunction with the duty on call team, to triage and manage patients requesting home visits, advice and urgent appointments by evaluating the patients condition, assessing and then implementing the appropriate programme of care including the option of face-to-face clinical consultations and nurse led prescribing in appropriate cases, within nurse prescribing guidelines and qualifications.

2. To maintain knowledge of medical developments within primary care and to ensure all new clinical guidelines are implemented and adhered to.

3. To undertake regular audits as required NHS England, Bristol ICB and identified key practice developments, particularly in relation to long-term condition management.

4. Assist in developing programs to fulfil PMS and QOF aims. Specifically:

    • Improved management of long-term conditions such as CHD, asthma, diabetes, COPD
    • Reduction in emergency medical admissions for patients with long term conditions
    • Reduction in smoking rates
    • Reduction in teenage and unwanted pregnancies
    • Improved sexual health
    • Improved management of minor illness
    • Improved physical health and healthy eating

5. To undertake home visits for selected patients with the agreement of the on-call team.

6. To be available to offer flu vaccinations or similar when conducting home visits.

7. To be involved in research projects as appropriate.

8. To promote a team approach to patient care through liaison with other Primary Health Care Team Members e.g. Treatment Room, District Nurses, Health Visitors, and Midwives.

9. To make efficient use of the EMIS system to record all consultations.

Person Specification

Qualifications

Essential

  • Relevant nursing/health degree and/or experience of advanced practice.

Desirable

  • Independent Prescribing qualification.
