Act as the first point of clinical contact for the patient, following patient referral. They will identify and respond to the immediate specialist palliative care needs, through direct liaison with the patient, carer or other involved health and social care professionals.

It is an exciting time at Trinity as we launch our new community services, which make up our new clinical hub. The new services have been developed to streamline the coordination of care for patients, address the evolving palliative and end of life care needs of our local population and maximise the flexibility and responsiveness we can offer.

We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of Triage Clinical Nurse Specialist within our First contact team.

This post requires you to act as the first point of clinical contact for the patient, following patient referral. You will identify and respond to the immediate specialist palliative care needs, through direct liaison with the patient, carer or other involved health and social care professionals.

Main duties of the job

  • To play a lead role in receiving and managing referrals made to the hospice.
  • Triage all incoming referrals considering patients stated symptoms to determine the appropriate level of care required to safely meet the patients specialist palliative care needs.
  • Utilise agreed resources and guidelines to effectively assess, prioritise, advise, schedule patient care management or refer patient when necessary to other appropriate health and social care agencies.
  • To ensure a timely, responsive, supportive and effective service for the benefit of all stakeholders involved in the patient referral.
  • Manage all aspects of the referral system to the hospice identifying appropriateness, urgency, and level of priority.
  • Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice, and ensure care is planned to meet the individualised and dynamic needs of the patients, and those close to them.
  • Use assessments and interventions as opportunities to train, coach and mentor clinical staff and visitors to the hospice about specialist palliative care practice.

Experience

Essential

  • Significant experience of providing direct patient services
  • Experience of maintaining effective inter-disciplinary team working structures and systems
  • Experience of managing challenging scenarios

Qualifications

Essential

  • Professional qualification and associated membership of nursing profession.
  • Registered with the NMC
  • Undergraduate learning in Palliative Care or working towards this.
