Job summary

We are looking to add a new care coordinator to our existing friendly, hardworking team. This is a non clinical role.

x1 vacancies for 22 hours per week

The Chislehurst Partnership is a friendly high achieving suburban partnership in the London Borough of Bromley with a countryside feel. Currently operating fromtwo sites (The Chislehurst Medical Practice & The Woodlands Practice)there are 9 partners, with 6 salaried doctors and full complement of nurses including ANP and HCA.

The successful candidates will be expected to work across the two sites. Days and hours to be discussed during interview.

Main duties of the job

Care Coordinators could potentially provide extra time, capacity, and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They will work closely with the practice team to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed. This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.

Chislehurst Medical Practice is part of the newly formed Chislehurst Partnership created in July 2022 with the merger of The Woodlands Practice and Chislehurst Medical Practice. Both are high performing, well established GP practices in Chislehurst who are passionate about providing a friendly, supportive and caring medical service to our population. The Practices are working towards moving into a purpose-built medical centre in the future but for the time being are operating from two premises. We have a combined patient list size of circa 25,000. Our team are friendly and hard-working and together we provide a high level of care to the local community. We are a well organised, progressive, forward-thinking Partnership. We are part of the Mottingham, Downham & Chislehurst (MDC) Primary Care Network and share across our practices a wide range of additional roles including, Clinical pharmacists, Physicians Associates, Pharmacy Technician, Physiotherapist, Social Prescribers, Mental Health Practitioner and Paramedic.

Job responsibilities

Duties of the role (not an exhaustive list) :

Carry out administrative work to support patient clinics such as vaccination clinics, and nurse clinics including setting up clinics, arranging appointments, liaising with other staff and patients when necessary;

Liaise with administration team with regards to practice indicators and targets;

Monitor Practices performance against contract targets and support work towards achieving those targets and feed back to the Contracts Manager;

Carry out administrative work for meetings

Carry out searches on the clinical computer system and analyse data, following practice procedures ensuring patients are encouraged to attend for clinical reviews

Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care;

Provide co-ordination and navigation for patients and their carers across health and care services working closely with social prescribing link workers, health and wellbeing coaches and other primary care professionals;

Work with patients, their families and carers, to improve their understanding of their condition.

Support patients to develop and review personalised care and support plans to manage their needs and achieve better healthcare outcomes.

Help patients to manage their needs by providing a contact to answer queries, make and manage appointments, and ensure that people have good quality written or verbal information to help them make choices about their care.

Assist patients to access self-management education courses, peer support, health coaching and other interventions that support them in their health and wellbeing, and increase their levels of knowledge, skills and confidence in managing their health.

Provide co-ordination and navigation for patients and their carers across health and care services. Helping to ensure patients receive a joined-up service and the appropriate support from the right person at the right time.

Proactively identify and manage a caseload, which will include patients with long-term health conditions, and where appropriate, refer back to other health professionals.

Support other members of the administration team

Maintain good communication with all members of the practice

Participate in training, as appropriate;

Work within organisation policies, procedures and guidelines.

  • Knowledge/familiarity with medical terminology
  • Awareness of clinical systems such as EMIS.
