Job summary

The post holder will be primarily responsible for providing care for our Elderly population with Long Term conditions including annual care planning and provision of some acute visits. These patients will be seen within the Practice setting, and in the Patients Home environment including Care Homes.

Main duties of the job

The Nurse will be attending patients who have been referred by a GP or by the Multi-Disciplinary Team (MDT) and will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner and referring back to the patients GP as needed. They will liaise with Service Providers, Carers, Social and Voluntary Services and the GP Out of Hours Service; as well as providing advice and support to Practice based and attached nursing staff; facilitating a seamless service for patients between Primary and Secondary Care.

The post holder must be able to work autonomously, be confident in providing high-quality, evidence-based clinical care and possess excellent interpersonal, organisational, communication and management skills

Job responsibilities

  • To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs, and other members of the ICC and primary care team.
  • Assess, diagnose and treat specific conditions in accordance with agreed professional protocols and guidelines and manages programmes of care and treatment for patients with Long term Conditions including frailty.
  • Proactively identify, help in diagnosing and manage treatment plans for patients at risk of developing a long-term condition.
  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
  • Work with patients in order to support adherence to prescribing treatments.
  • Actively engage patients and carers in management of their health, promoting health and wellbeing and self-care.
  • Discussion with patients named GP as required to formulate the patients care plan.

Skills, Knowledge and Attributes

Essential

  • Knowledge of accountability of own role and other roles in a nurse-led service
  • Knowledge of health promotion strategies
  • Knowledge of clinical governance issues in primary care
  • Ability to change care management skills to support patients
  • Excellent verbal communications skills, able to communicate difficult messages to patients and families
  • Good written communication skills, clear concise record keeping
  • Time management, able to organise workload
  • Knowledge of national policy standards which inform and have implications for Practice , NICE, NSF, QOF

Desirable

  • EMIS clinical system
  • Knowledge of local and national health policy
  • Wider health economy awareness

Qualifications

Essential

  • Registered (first level) Nurse
  • Current NMC registration
  • Evidence of post registration education

Desirable

  • Educated at diploma, degree level
  • Independent prescribing qualification

Other

Essential

  • Must be flexible in working hours / able to work at desired times including evenings and Saturdays
  • Own transport

Experience

Essential

  • Minimum 3 years experience
  • Portfolio evidence of continuous professional development
  • Ability to work as part of a team as well as an autonomous practitioner
  • Multi-disciplinary and multi agency working

Desirable

  • Primary and community care experience
  • Experience of nurse led management of long term conditions
  • Experience of developing and implementing protocols and clinical guidelines
