An exciting opportunity is available to join Meir Primary Care Network as our Advanced Care Practitioner - Nurse.
The right candidate will be motivated, enthusiastic and experienced Practitioner who is able to work effectively within our clinical team to undertake patient assessment, diagnosis and clinical management.
Main duties of the job
The successful candidate should be an experienced practitioner who, acting within their professional boundaries and competencies, will provide care to the presenting patient, from initial clinical assessment, through diagnosis, treatment, and an evaluation of care. They will demonstrate safe and effective clinical decision making and critical thinking. Above all they will be a caring and compassionate clinician who puts the needs of their patient at the heart of everything they do.
About us
Meir PCN is a membership comprising of six practices - Adderley Green Surgery, Meir Medical Practice, Meir Park & Weston Coyney Medical Practice, Dr Miles & Valasapalli, Trinity Medical Practice and Willow Bank Surgery - covering the Meir, Longton & Blythe Bridge areas of Stoke on Trent.
We currently have a PCN population of approximately 38000 patients and the PCN staff work in conjunction with the local general practices, local services and other organisations to support both local and national priorities.
We take immense pride in our PCN and how we are providing excellent support and care to our practice colleagues and patients.
Our goal is to improve health & care locally and to meet everyday health and care needs for patients by connecting primary care systems and using creative thinking to develop, improve and support great local services.

Job responsibilities
Recognise
and work within own competence and professional code of conduct as regulated by
the NMC
Assess,
diagnose, plan, implement and evaluate treatment/interventions and care for
patients presenting with an undifferentiated diagnosis
Assess
and triage patients, including same day triage, and as appropriate provide
definitive treatment (including prescribing medications following policy,
patient group directives, NICE (national) and local clinical guidelines and
local care pathways) or make necessary referrals to other members of the
primary care team
Manage
undifferentiated undiagnosed condition and identify red flags and underlying
serious pathology and take appropriate action
Use
complex decision making to inform diagnosis, investigation, complex managements
of episodes of care within a broad scope of practice
Actively
take a personalised care approach and population centred care approach to
enable shared decision making with the presenting person
Clinically
examine and assess patient needs from a physiological and psychological
perspective, and plan clinical care accordingly
Assess,
diagnosis, plan, implement and evaluate interventions/treatments for patients
with complex needs
Proactively
identify, diagnose, and manage treatment plans for patients at risk of
developing a long-term condition (as appropriate)
Diagnose
and manage both acute and chronic conditions, integrating both drug- and
non-drug-basedtreatment
methods into a management plan
Prescribe
and review medication for therapeutic effectiveness, appropriate to patient
needs and in accordance with evidence-based practice and national and practice
protocols, and within scope of practice
Work with
patients in order to support compliance with and adherence to prescribed
treatments
Provide
information and advice on prescribed or over-the-counter medication on
medication regimens, side-effects, and interactions
Prioritise
health problems and intervene appropriately to assist the patient in complex,
urgent or emergency situations, including initiation of effective emergency
care
Support
patients to adopt health promotion strategies that promote healthy lifestyles,
and apply principles of self-care
Support
and manage health needs of women presenting for family planning, cervical
cytology or sexual health consultation
Implement
and participate in vaccination and immunisation programmes for both adults and
children
Communicate
effectively with patients and carers, recognising the need for alternative
methods of communication to overcome different levels of understanding,
cultural background and preferred ways of communicating
Take
responsibility for own learning and performance including participating in
clinical supervision and acting as a positive role model
Actively
promote the PCN as a learning environment, encouraging everyone to learn from
each other and from external good practice
Prioritise
own workload and ensure effective time-management strategies are embedded
within the culture of the team
Discuss,
highlight, and work with the PCN to create opportunities to improve patient
care
Manage
and lead on the delivery of specifically identified services or projects as
agreed with the PCN
Apply
infection-control measures within the practice according to local and national
guidelines
Act as a
role model in the observance of equality and diversity good practice
Two clinical references will be requested as part of pre-employment checks

Experience
Essential
- 5 years post registration experience including 6 months experience in a GP practice/ Walk in Centre/ Urgent Care Centre
- Nurse-led management of minor illness, minor ailments, and injuries
- Nurse-led triage
- Compiling protocols and clinical guidelines

Qualifications
Essential
- Registered first level nurse
- MSc in a nursing related subject
- Graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported)
- Both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice.

- Non-Medical Prescriber
Desirable
- Level 3 Extended brief interventions (NICE guidelines)
- NMCs Academic Assessors, Practice Supervisors, and Practice Assessors accreditations
