Main duties of the job

Work both with individuals that are presenting with a first presentation of mental health need and those with on-going mental health needs that do not require support from the core mental health team, and to carry out annual reviews as needed.

To undertake physical health checks for those patients on the SMI register and facilitate follow up support as required.

Work as an autonomous practitioner to deliver a range of mental health interventions with support from the senior mental health practitioner and the wider allied core mental health team as required.

Person Specification

Education/ Qualification/ Training

Essential

  • oRegistered Mental Health care professional, with current registration with relevant body e.g NMC or HCPC
  • oEvidence of post registration continued professional development, qualifications or equivalent experience

Desirable

  • oEducated to degree level or equivalent experience

Experience

Essential

  • oMinimum of one year post-registration experience and completion of preceptorship
  • oPrevious experience of working in a multi-disciplinary team in primary or community setting
  • oExperience of multiagency working
  • oPrevious experience of working with people experiencing complex mental health presentations
  • oExperience of undertaking specialist mental health assessments and assessment of need.
  • oExperience of comprehensive assessment of risk and developing risk management strategies

Knowledge and Skills

Essential

  • oGood knowledge of current and relevant local and national agendas that impact upon PCNs, mental health services and the wider health and social care system and the ability to understand and disseminate this knowledge to others
  • oSound knowledge and understanding of the Mental Health Act (1983/ 2007/ 2014/5), Care Act (2014), relevant national standards and other relevant national policies.
  • oExcellent knowledge & understanding of recovery principles and NICE guidance in relation to common mental disorders and the ability to ensure that evidence based practice with clear clinical outcomes is embedded into practice.
  • oDemonstrate a knowledge base for substance use and associated risks.
