Job summary

As a Health Care Support Worker, you'll be working within an Integrated Care Team in the community delivering care to housebound patients in the community in the West New Forest area (Ringwood and Fordinbridge). You would be working under the supervision of a Community Nurse.

This is a diverse role offering responsibility for maintaining patient's health and well-being in their own homes, with the aim of avoiding any unnecessary admissions into Acute Hospital In-Patient setting.

We offer a wide range of different clinical experiences. Opportunities are available for a motivated individual to build their skills in a variety of areas.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

Qualifications

Essential

  • *NVQ 3 in health care related subject or willingness to undertake

Desirable

  • *First aid certificate

Experience

Essential

  • *Relevant experience as a care assistant in a health care environment.
  • *Evidence of delivering care delegated from a range of registered professionals, in particular nursing.
  • *Evidence of ability to implement prescribed programmes of care for a range of conditions.

Desirable

  • *Community experience
  • *Experience in carrying out clinical skills for example, blood pressure, urinalysis
  • *Previous experience in group work, for example ability to conduct patient education sessions
  • *Previous experience of discharge co-ordination in hospital setting
  • *Experience in supporting and supervising students or staff
