East Twilight/ Night response HCSW

An exciting opportunity has arisen within Southern Health Foundation Trust to appoint an inspirational and enthusiastic HCSW to join the out of hour's service. Working Twilight, the shift times are 17:00 until 23:30 and nights 22:00 until 04:30. the teams cover all the GP practices across the Fareham and Gosport and East Hants area. You will be working alongside a trained nurse in one of the areas listed above.

The work is varied and interesting including, end of life care, catheter management, falls support, Diabetes management and wound care. You will be responding directly to client needs and liaising with the Community care teams in the area. You will also be liaising with the out of hours GP services and SCAS providing care within patient's homes and care homes and responding to visits to support hospital discharges and to prevent hospital admissions as part of our urgent care response service.

We are looking for motivated and dynamic people who have excellent communication skills, basic IT skills and the ability and knowledge to work autonomously in the community. This position is suitable for a HCSW who holds NVQ level 2/3 in care and the care certificate would be desirable to face the challenges of out of hour's provision.

Access to a vehicle and possession of a full driving license is essential.

Main duties of the job

The post holder will have responsibility for participating in the delivery of patient care under the supervision of the Community Care Team.

Work under supervision following written care plans and care pathways devised by the Registered Nurse.

Follow prescribed care plans and treatment protocols.

Follow HPCT Policies and Procedures.

Has knowledge and skills to carry out prescribed tasks and will report to Registered Nurse any variances.

Delegated responsibility for Patient Support Teams administrative tasks.

Provide general administration support to the team.
