Job summary

We are looking for enthusiastic and motivated individuals to join our team on Ward 0C Surgical/ENT ward at The Grange University Hospital. Applications are invited from those who hold NVQ Level 2 or 3 qualifications and those with previous experience in an acute setting.

The successful candidates will possess excellent communication and interpersonal skills. The willingness and ability to work in a constantly changing environment is also an essential requirement.

Main duties of the job

The HCSW will provide high quality, consistent and cost-effective health care for patients with acute physical and psychological needs. You will need to demonstrate a commitment to delivering high quality person centred care. You will need to have excellent communication skills and the ability to work effectively with patients, carers and other professionals. You must be an excellent team player, have high levels of motivation and enthusiasm for working in a demanding but rewarding service. You will be joining a team that is passionate and focused on achieving excellence in care delivery, for all patients in this area of specialised nursing.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Job responsibilities

PRINCIPLE ACTIVITIES

Clinical

Successfully complete a locally approved orientation programme. To support a clinical environment that is patient and public focused. To assist patients achieve physical spiritual and psychological comfort. To participate in the delivery of patients care and undertake clinical duties that are allocated and directly supervised by a registered nurse. To share with Team Leader information relating to the patients care. Assist with patients personal hygiene needs. Assist with patients nutritional needs to include assistance with feeding. To report any changes in a patients condition to the nurse in charge and respond to clinical urgencies. To utilise customer care skills when caring for patients and relatives. To escort and accompany patients needing to attend appointments to departments/hospitals within the Trust To maintain and protect confidential information relating to patients. To undertake any other duties consistent with the overall grade of the post. To establish and maintain good working relationships with patients, relatives and members of the inter-disciplinary team. To work as a key member of the team with the patient to reach your negotiated goals. To contribute to the promotion of health and prevention of illness, as directed by the Registered Nurse.

Managerial

To act always in accordance with Trust policies and procedures, To ensure all accidents, incidents and complaints are reported and to assist in the investigation of them. To work with the Modernisation Team to improve service and be positive champion for any change proposed. To attend and participate in ward/department/area meetings. To report any concerns regarding patient care or environment to the Nurse in Charge. To be aware of Clinical Governance and act within the Trust's Clinical Governance framework. To work within agreed protocols, national guidelines and Trust wide policies and procedures. To raise awareness of the Trusts dignity at work policy and the procedures for dealing with these issues.

Educational

To undertake and participate in statutory training and in serviceProgrammes, NVQ Level 2 or 3 in Health and Social Care or equivalent To participate in annual personal review being cognisant of service needs.

Speciality Specific Activities

To perform general observations, to include pre and post operation. Checking patient has a wristband and that it is legible. Maintenance of a clean environment and the equipment used. Maintain stock levels and the putting away of delivered stores. Be proficient and perform record keeping where they are involved in taking the observations e.g. fluid balance, observations, stool charts etc. Taking of BMs for Blood Glucose monitoring.
