To support the Hepatology Clinical Nurse Specialists and the Hepatology Consultants in providing optimum patient care; this will be mainly in the outpatient clinic setting but may include some ward visits.

Main duties of the job

Main Duties/ Responsibilities

  1. As part of our commitment to patients and delivery of a world class service for all we have created the UHCW Improvement (UHCW) System in partnership with the Virginia Mason Institute in Seattle; this involves a structured approach to removing waste and putting the patient first using a lean management system and methodologies. Our culture and ways of working reflect and embed the practices and methodologies of UHCW.

  1. Responsible for own actions, ensuring all patient interactions/observations are communicated to the appropriate healthcare practitioner.

  1. Undertake agreed clinical and non-clinical tasks, to include, after the appropriate training and supervised practice liver fibro scans on patients identified by either the medical team or hepatology Clinical Nurse Specialists, to document the readings in the appropriate identified location to ensure results are communicated in a timely manner.

  1. Organise own daily workload to include ensuring availability of the fibroscan equipment for the appropriate clinics.

  1. Maintain a clean, safe work environment ensuring adherence to Trust standards of cleanliness, hygiene and infection control at all times. For example to ensure the fibroscan equipment is kept clean and serviceable

Job description

Job responsibilities

  1. Participate in conjunction with the Hepatology CNSs the collection of data. Participate service Audits.

  1. Assist in Training delivery and Education within the service including health and wellbeing promotional days. Contribute to the development and delivery of structured training programmes to raise alcohol awareness and support practice among the wider multi-professional team.

  1. To support nursing staff who provide Identification and Brief Advice to patients when they require further advice

  1. 9. Develop skills in the recognition of the need for:

i)Referral of patients to specialist alcohol services, where appropriate.

ii)Referral of patients for medical/surgical or psychiatric assessment where appropriate

iii)Referral of patients to other specialists for advice and treatment.

iv)Liaison with the voluntary agencies and the alcohol services in the community.

  1. Collaborate in the provision of advice and support to other health care professionals and external agencies including Primary Care Services, including social care, and housing.
