We are looking for Band 6 Occupational Therapists to join our Multidisciplinary Team (Discharge To Assess Pathway 3).

As a Band 6 Occupational Therapist, it is an exciting time to be part of our MDT that assesses patients who are discharged from hospital to a temporary community bed.

The Pathway 3 Therapy Team consists of Occupational Therapists, Physiotherapists and Rehab Assistants. We work directly with Pathway 3 Co-coordinators, Social Workers, Nurse Clinicians and other specialist community services as part of a holistic MDT. We provide assessments to patients in a supported environment with a view to them returning to home living as independently as possible.

You will be supported full time by an experienced Band 7 Senior Practitioner and have opportunity to further develop your clinical and operational skills through training, supervised practice and professional support.

You will need to be a strong team player, an excellent communicator, adaptable, committed, well organised and work well under pressure. This position requires you to be a car driver, have access to your own vehicle and be able to travel around the localities of Manchester.

Main duties of the job

This exciting opportunity for a Band 6, a chance to promote Occupational Therapy, gain confidence and professional skills and knowledge in other clinical areas allowing you to be a dynamic practitioner, use various models of practice and the chance to be truly holistic within your role.

The post holder will be responsible for providing a comprehensive assessment of patients having been discharged from hospital via the D2A Pathway 3 to a community bed- based service. The team is multidisciplinary, including other AHP staff, nurse clinicians and social workers.

You will be expected to work collaboratively with members of the MDT team, act as an autonomous practitioner and champion the role of Occupational Therapy within this service. The ability to problem solve and think on your feet is an integral part of the day to day requirements of this role

The purpose of the role is to provide therapy assessments and professional input to multi-disciplinary decisions about long term care needs with a primary focus on supporting people to return to their own homes wherever possible, avoiding long term care placements

D2A beds are commissioned primarily in Trafford but can be across Greater Manchester and you will be required to travel to a range of care homes to undertake functional assessments, rehabilitation interventions, recommendation of equipment and to contribute to home assessments.

Qualifications

Essential

  • HCPC Registration
  • BSc / MSc in Occupational Therapy (or equivalent)

Desirable

  • Leadership Course / Qualification

Experience

Essential

  • Relevant NHS experience working in an MDT
  • Broad experience as a Band 5/6 core clinical areas
  • Experience of teaching others, mentoring students and supervising junior staff members
  • Experience of urgent assessments and developing and delivering care plans to meet these needs
  • Relevant community experience

Knowledge

Essential

  • Knowledge of current health policy and relevance to the delivery of health care service
  • Knowledge and understanding of complex medical, social and psychological needs of older adults
  • Knowledge of evidence based practice
  • Knowledge of managing difficult situations and conflict resolution

Skills

  • Able to fully complete tasks within agreed deadlines
  • Ability to work to time pressure as part of caseload management
  • To carry out risk assessments and ensure effective risk management strategies
