Main duties of the job

As an Advanced Clinical Practitioner (ACP), you will play a key role in providing advanced clinical assessment, diagnosis and treatment to our patients. Working collaboratively with our multidisciplinary team, you will have the opportunity to manage a diverse caseload, including acute and chronic conditions, minor injuries and long term health needs.

Ash Grove Medical Centre is CQC rated 'Good' across all five areas. We are a large GP Practice with 13,000 patients based in Knottingley with a branch surgery in Eggborough.

Ash Grove Medical Centre is led by four GP Partners working along side the Practice Manager, supported by our clinical and non clinical team.

Key responsibilities

  1. Conducting thorough assessments, physical examinations and diagnostic tests.
  2. Formulating differential diagnoses and developing evidence based management plans.
  3. Prescribing medications and treatments in accordance with clinical guidelines and protocols.
  4. Providing health education and promoting self-management strategies to patients and their families.
  5. Collaborating with GP's, nurses and other healthcare professionals to optimise patient care and outcomes.
  6. Participating in clinical audits and continuous professional development activities.

Job description and responsibilities

  1. Ensure that confidentiality is maintained at all times, with a duty not to disclose ant confidential information after employment ends.
  2. To work independently using own initiative and is accountable for own professional actions.
  3. To assess, diagnose, plan, implement and evaluate treatment/intervention and care for patients present with an undifferentiated diagnoses.
  4. To assess, diagnose, plan, implement and evaluate treatment/intervention and care for patients with complex needs.
  5. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition.
  6. Clinically examine and assess patients needs for a physiological and psychological perspective and plan clinical care accordingly.
  7. Diagnose and manage both acute and chronic conditions. integrating both drug and nondrug based treatment methods into a management plan.
  8. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols and within scope of practice.
  9. To work with patients to support compliance with and adherence to prescribed treatments.
  10. Provide information and advice on prescribed or over the counter medication on medication regimens, side effects and interactions.
  11. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations including initiation of effective emergency care.
  12. Support patients to adopt health promotion strategies to promote healthy lifestyles and apply principle of selfcare.
  13. Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for mental health.
  14. Carry out the partners instructions and requests.
  15. All staff are required to take their share in providing cover, when necessary, for colleagues who may be absent as a result of holidays or illness.
  16. Maintain a smart appearance and be helpful wherever possible.
  17. All staff should be conversant with and adhere to all written guidelines and protocols.

Extra duties and responsibilities

  1. To provide accurate, contemporaneous and complete records of patients consultations, consistent with legislation, policies and procedures.
  2. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  3. IPC compliance and waste disposal.
  4. Deliver care according to NSF,NICE guidelines and evidence based care.
  5. Maintain practice effective communication within practice environment and with external stakeholders.
  6. To raise any significant events and discuss learning opportunities in clinical and MDT meetings.
  7. Target outstanding QOF and arrange for this to be completed.
  8. Liaise with colleagues and ensure hand over is carried out.
  9. All patient identifiable data is locked away at the end of the day.
Organisational and Statutory requirements

The post holder must be aware of the responsibility placed on him/her under the Health & Safety at work Act 1974 and ensure that agreed safety procedures are carried out in order to maintain a safe environment for employees, patients and visitors.

All employees should understand that it is their personal responsibility to comply with all other organisational and statutory requirements eg confidentiality, equal opportunities, child protection, in house complaints procedure, fire alarm protocols and all protocols that relate to their specific role etc.

Understand and apply legal issues that support the identification of vulnerable and abused children and adults and to be aware of statutory / vulnerable patients health procedures and local guidance.

Other information

Overtime, to cover annual leave and sickness is an essential part of the job.

Health & Safety

The post-holder will assist in promoting and maintaining their own and others health, safety policy, the practice health and safety manual, and the practice information control policy and published procedures. This will include:

Using personal security systems within the workplace according to practice guidelines.

Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.

Making effective use of training to update knowledge and skills.

Using appropriate infection control procedures, maintaining work area in a tidy and safe way and free from hazards

Actively reporting of health and safety hazards and infection hazards immediately when recognised.

Keeping own work areas and general/patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role

Undertaking periodic infection control training (minimum annually)

Person Specification

Qualifications

Essential

  • Registered nurse with current NMC registration. Masters degree in Advanced Clinical Practice or Advanced Nursing Practice. Prescribing qualification and experience.

Desirable

  • Teaching/mentoring experience and/or qualification.

Experience

Essential

  • Understanding of evidence based practice.
  • Polite, confident, flexible and cooperative. Excellent interpersonal, verbal and written communication skills. Reflective practitioner, motivated and a forward thinker, willingness to undertake training at a higher level if necessary. Problem solver with the ability to process information accurately and effectively, interpreting data as required.
  • Understanding of the national and local health agenda, awareness of legal and ethical issues of the role. Skills in clinical examination and diagnostic techniques, empathetic and sensitive in distressing situation as well as the ability to work under pressure/ in stressful situations.
  • Extensive experience working autonomously in primary care or community settings. Commitment to continuous professional development and evidence based practice.

Desirable

  • Ability to be self motivated. Ability to think strategically. Experience of use of medical software package- SystmOne/Ardens.
  • Evidence of clinical audit, evaluation research and implementation of evidence based practice. Interpreting and implementing local and national policy agendas for health.
