Job summary

Park Farm Medical Centre & Vernon Street Surgery in Derby have a fantastic opportunity for an experienced practice nurse. The role ideally is full-time, but we are open to negotiate slightly less hours for the right candidate. We are looking for a registered nurse with a minimum of 18 months experience in delivering chronic disease management for Diabetes and ideally for COPD, Asthma, CHD & CVD.

We plan to interview on 3rd, 4th, 5th June 2024.

Main duties of the job

To be fully proficient in Chronic disease management, vaccinations and immunisations, cervical screening, contraceptive reviews, wound care, as well as other general treatment room duties.

To work as part of a cohesive team of fellow professionals, maintaining high standards and delivering quality care to patients.

About us

We are an enthusiastic team who are passionate about providing high quality care to our patients. We offer a supportive and nurturing environment for our nursing team which comprises four Practice Nurses, one Healthcare Assistant and one Advanced Nurse Practitioner.

Our nursing team is well supported by our GPs, Admin and Reception Teams. We provide protected admin time to allow for additional tasks to be completed and follow-up to be undertaken.

Our largest surgery is based at the Park Farm Shopping centre in Allestree, with our branch surgery on Vernon Street, in the Friar Gate area of Derby. The practice has nearly 13,000 registered patients across the two sites.

The main responsibilities of the role include:

  • Chronic disease management for Diabetes
  • Ideally chronic disease management for COPD, Asthma, CHD & CVD
  • Lung function testing including Spirometry and FeNO
  • Health promotion
  • Delivery of national immunisations and vaccinations programmes, including participation in annual flu and bi-annual covid campaigns
  • Cervical cytology screening
  • Wound care and management
  • Contraception reviews
  • Travel health advice and immunisation
  • Responsibility for maintaining CPD, keeping updated with evidence-based practice and participation in annual appraisals
  • To provide support and guidance to Student Nurses on placements
  • Knowledge of adult and child safeguarding responsibilities and awareness of local referral procedures
  • Adherence to Infection Control processes and procedures

Person Specification

Qualifications

Essential

  • Current NMC Registration.
  • Nursing degree or other equivalent nursing qualification.
  • Diabetes Diploma or equivalent training / qualification.

Desirable

  • Basic Life Support & Anaphylaxis Update (will need renewing if expired).
  • Asthma Diploma or equivalent.
  • COPD Diploma or equivalent.
  • ARTP Spirometry Performing and ideally Interpretation.
  • Registration on the Cervical Cytology Sample Taking Register.
  • Travel Health 2 day Initiation Certificate & updates.
  • Childhood immunisation 2 day initiation Certificate.

Experience

Essential

  • 18 Months+ experience delivering Chronic Disease Management for Diabetes.
  • Positive attitude, willing to work as part of a cohesive and supportive team.
  • Good organisational skills, ability to multitask and prioritise.
  • Excellent communication skills & patient care.
  • Strong team working skills, able to work within a multidisciplinary team.

Desirable

  • Experience of chronic disease management for COPD, Asthma, CHD & CVD.
  • Experience in Spirometry and FeNO.
