Job summary

Thornbury Medical Centre looking for an experienced Practice Nurse to Lead our nursing team and join our vibrant and friendly Practice.

We pride ourselves on delivering high quality holistic care to our patient population and have a culture in learning and teaching in all we do. We are looking for a forward thinking experienced Practice Nurse who has the qualifications and/or experience to support our patients with long term health conditions and be comfortable with the ever-changing day to day world of General Practice.

Main duties of the job

The Lead Practice Nurse will be responsible for the management of the practice nursing team, consisting of nurses and HCA.

They will be responsible for several clinical areas such as infection prevention and control, chronic disease management, health promotion, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

About us

We are an average sized practice with just over 7,000 patients. GPs, Advanced Clinical Practitioner, Pharmacist, HCA and Physician Associate and other health professionals.

We have an admin and reception team that supports the clinical team and patients.

We are part of Bradford PCN 5 care network and work closely with our neighbouring practices.

Job responsibilities

  • Leading the team to meet patient needs including QoF outcomes and other local priorities.
  • Work with our Patient Support team to ensure the recall system reviews patients with the right people at the right time.
  • Be the voice of the nurse team within practice management.
  • Support and mentor the professional development of your colleagues
  • Manage the Nurse Team - arranging rotas and cover.

Person Specification

Experience

Essential

  • Experience of leading a nursing team.
  • Experience of chronic disease management.
  • Experience of working autonomously.
  • Experience of infection prevention and control measures.
  • Experience with audit and able to lead audit programmes.
  • Experience with clinical risk management.

Desirable

  • Experience of working in a primary care environment.
  • Experience of working as a practice nurse or community nurse.
  • Experience of managing elderly and housebound patients.
  • Experience of quality initiatives, i.e., benchmarking.

Qualifications

Essential

  • Registered Nurse (Nursing and Midwifery Council).
  • .

Desirable

  • Mentor or Teaching Qualification.
  • Post Graduate Diploma or Degree (Chronic Disease Management).
  • Nurse Prescriber.
  • Family Planning Qualification.
