Main duties of the job
- Demonstrate evidence based nursing , when initiating individualised plans of care.
- Manage and prioritise own workload, ensuring care delivered is patient focussed.
- Establish effective communication processes with patients, families, colleagues, and the broader multi-disciplinary teams and external agencies.
- Support colleagues in providing high-quality palliative and end-of-life care, including pre and post bereavement support.
- Recognise the signs and symptoms of palliative medical emergencies and act promptly to escalate concerns.
- Recognise the importance of debriefs and supervision in self-care management and encourage colleagues to do the same.

Job responsibilities
To work as a member of the Hospice
multi-disciplinary care team to work towards the achievement of the aims and
objectives laid down in the philosophy of the Keech Hospice Care.
To ensure each patient and family receives
a holistic assessment, their care needs identified, recorded and a plan of care
implemented and evaluated in collaboration with other agencies and services
that may be involved.
To work flexibly across Adult Care Services as
required to ensure continuity of patient care (Adult inpatient unit, Wellbeing
Centre, Community Liaison, Community).
Person Specification
Experience
Essential
- NMC Registered Adults Nurse Level 1 with experience of caring for patients with a palliative diagnosis.
- Ability to work autonomously and collaboratively with the multi-disciplinary team.
- Understanding and adherence to relevant legislation, professional standards, and national strategy.
- Effective communicator with great interpersonal skills.
- Motivated to develop a career in palliative care.
Desirable
- Palliative Care qualification or attendance at Palliative Care training sessions
- Practice Supervisor
- Advanced Communication skills training
- Experience of caring for patients at end of life
