Job summary

We have an exciting opportunity for a Primary Care Liaison Practitioner in our Richmond Wellbeing Service.

The Richmond Wellbeing Service is committed to work in partnership with service users and their carers, as well as with other statutory and non-statutory services. Our aim is to provide strengths based and recovery orientated interventions in order to maximize the quality of life and wellbeing of service users.

The post holder will work exclusively within the Primary Care Liaison Team (PCL) in the London Borough of Richmond. The PCL team is one part of the Richmond Wellbeing Service, working alongside the psychological therapies team, provided within the IAPT Talking Therapies framework.

Main duties of the job

The post holder will promote clinical excellence and expertise in providing care for patients with mild to moderate mental health difficulties and support the delivery of an agreed and coherent, collaborative and co-produced care plan, in keeping with care pathways, operational policies and NICE guidelines. The post holder will deliver services to maximise engagement of service users and support them in achieving their recovery goals. The post holder will be involved in service development and implementation of the requirements of the NHS Long Term Plan and the National Service Framework for Mental Health Standard 2.

Job description

Job responsibilities

  1. To promote clinical excellence and work within the framework of evidence based medicine and best practice.
  2. To liaise effectively and pro-actively with GP Practices, Integrated Recovery Hubs/Single Point of Access (SPA), and other voluntary organisations, creating positive working relationships and positive service experience for services user, carers and other professionals.
  3. To encourage and support service users and carers involvement in their care and treatment.
  4. To work as an effective member of the multi-disciplinary team ensuring effective communication with other team members, other professionals and members of the community.
  5. To ensure service users receive a timely and effective Primary Care Mental Health intervention, as agreed by pathways, protocols and NICE guidelines.
  6. To ensure referrals are processed appropriately.
  7. To deliver comprehensive biopsychosocial assessments, diagnostic formulations and clinical reviews and clinical interventions with and for service users
  8. Offering medication reviews and advise to patients, carers and GPs
  9. To ensure effective transfer of secondary care service users to Primary care, including service users on depot medication (Depot Enhanced Service). This transfer will be based on detailed clinical reviews and systematic analysis of patients needs which will be undertaken by the post holder.
  10. To support GPs and other professionals in primary care with assessing and treating patients by role modelling, facilitating, and training.
  11. To deliver consultation, advice and clinical supervision to other members of the clinical team regarding Primary Care service provision.
  12. To lead multi-disciplinary and multi-agency clinical meetings when required

Person Specification

Experience

Essential

  • Extensive post qualification experience in relevant field in Mental Health
  • Substantial experience of working autonomously and independently in community settings.
  • Substantial experience in community caseload management and mental health liaison.
  • Experience in working with diverse agencies in a complex environment
  • Experience of working with primary care.

Desirable

  • Experience of working with people with chronic physical health conditions

Training & Qualifications

Essential

  • Recognised mental health qualification in nursing, social work, occupational therapy or psychology
  • Current Professional Registration
  • Evidence of continual professional development

Desirable

  • Qualification in a psychological treatment modality
  • Approved to prescribe medication

Knowledge & Skills

Essential

  • Ability to provide structured and focused liaison and consultation to other professionals
  • Ability to organise and prioritise own workload
  • Excellent time management skills
  • Knowledge and understanding of mental health and co morbid chronic physical health issues in Primary Care
  • Good knowledge of psychopharmacology
  • Detailed knowledge of ICD-10 for mental disorders
  • Ability to communicate effectively both verbally and in writing about a range of complex issues
  • Highly developed problem solving and conflict resolution skills
  • Ability to use current IT systems and packages for data entry and analysis
  • Ability to analyse and present data in meaningful way
