As aHospital Liaison Worker, you will play a crucial role in enhancing the engagement of individuals with alcohol or drug concerns within our treatment services.

Job description

Job responsibilities

Working hours: 9am-5pm Monday to Friday.

Your responsibilities will include:

  • Providing 1:1 psychosocial intervention to repeat attending patients at on the wards and in ED, engaging them in our wider drug and alcohol treatment services in Haringey .
  • You will carry out initial screening and triage assessment for new clients, and provide crisis intervention and duty services on a rota basis.
  • Liaising with other services including mental health, primary care, disabilities, social care, and any other relevant services in response to service user needs.
  • Monitoring, reporting on, and improving outcomes across a range of measures, particularly to reduce preventable GP attendances, urgent care centre attendances, A&E attendances, hospital admissions, and ambulance call-outs for the service users in the frequent attender cohort.
  • Conducting hospital ward rounds to identify those with alcohol-specific needs and engage them in treatment.
  • Traveling to various locations within Haringey, to make contact and build up relationships with the hospital frequent attenders through regular and consistent contact episodes and motivational interviewing oriented interventions.
  • Advocating on behalf of service users in obtaining assistance with housing, benefits, mental and physical health care, and access to substance misuse treatment services at whichever tier is appropriate.
  • Assisting service users in developing strategies, which improve self-esteem, social skills, and psychological wellbeing that supports their rehabilitation from alcohol misuse.

The salary advertised above demonstrates the full earning potential for this role, starting salaries will allow for pay progression within the pay band.
