Main duties of the job
Within this patient facing role you will provide person-centred, coordinated care for our patients. You will support the practice medicines optimisation work and have input into the following areas:
Structured Medication reviews
Oversight of repeat prescribing requests and systems.
Management of medicines following admission and discharge to hospital
Acute prescribing for minor ailments and common illnesses
Supervision of monitored dosage systems processes.
Conducting medication reviews and giving advice to patients
Management and support of patients with long term illness
Clinical reviews of care home patients.
Polypharmacy medication reviews
Provide primary support to practice staff for prescription and medication queries.
In addition, the post holder will be involved in encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and with other stakeholders and promoting good practice in line with therapeutic developments.It would be an advantage if you were an independent prescriber but training and support to become an independent prescriber will be available if required.

Job description
Job responsibilities
The post holder
is a clinical pharmacist who is used to acting within their professional
boundaries, who will work alongside our team clinicians and other clinical
pharmacist at our GP practice.
They will
improve patients health outcomes and the efficiency of the primary care team
by providing direct, accessible and timely medicines and expertise including
face to face contact with patients.
They will be an
integral part of the general practice team, as well as part of a wider
pharmacist network and multi-disciplinary team. This includes being a conduit
of patient medicines information into and out of hospitals. They will work to
optimise medication issues to improve patient care and safety, and support
clinical staff in the management of patients.
As part of
their employment, they will achieve qualifications from our local accredited
training pathway, including independent prescribing, equipping the clinical
pharmacist to be able to practise and prescribe safely and effectively in a
primary care setting.
A supervision and mentorship network will be provided
to support the clinical pharmacist.
Person Specification
Qualifications
Essential
- GPhC registered pharmacist
- Enrolled in, or has qualified from, an approved 18-month training pathway or equivalent that equips them to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE clinical pharmacist training pathways)
Desirable
- Hold or be working towards an GPhC independent prescribing qualification
- Able to deliver the key responsibilities outlined in section B1.3 of the Network Contracted Enhanced Service Specification 2023-24
Experience
Essential
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
- Experience and an awareness of common acute and chronic conditions that are likely to be seen in general practice
Desirable
- Experience working as a pharmacist within a general practice setting

- Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face
