Job summary

Hospice Community Services (HCS)provides palliative and end of life care to people in their own homes. The service has grown since its inception back in 2003 (formally known as Hospice at Home), with a number of services now under the umbrella service of HCS including a 24 hour 7 day a week OneResponse Service, Hospice Rapid Access Service, Frailty and Care Home Palliative Support Service, Clinical Nurse Specialist Service and Hospice at Home. HCS has been recognised as being an integral service working within the wider Mid and South Essex Health and Social Care system.

With the increased growth in the service, a change to the service model for HCS is being implemented to ensure the service remains fit for the future responding to changes in activity and demand whilst ensuring that it aligns with the core values and standards of care held within the Hospice

Working with the Director of Care and Commissioning and the Assistant Director of Hospice Community Services and Community Partnerships this role is required to project manage this service development project. Responsibilities will include planning, executing and supervising the project to meet the managements strategic objects and outcomes ensuring project delivery within budget and on time.

Main duties of the job

Key Responsibilities

  • To set and agree a mobilisation plan for the project, identifying core project requirements, designated leads, stake holders and delivery timelines.
  • Develop a detailed project plan to guide and inform the projects strategic steering group regularly reviewing and revising the plan based on changing needs and requirements within the project
  • To work with designated leads to plan subsection work and work plans associated with the project ensuring adherence to service milestones, setting timelines and objectives and escalating risks to the Project Lead.
  • To actively monitor the progress of the project, strategically and operationally against service milestones, escalating concerns and risks to the Project Lead and Project Steering Group and making recommendations for consideration where corrective action may be required
  • Manage project budget and project resource allocation
  • Plan and schedule project timelines

Person Specification

Experience

Essential

  • Experience working in a health care setting
  • Demonstrable experience of data collation analysis
  • Excellent communication and and negotiation skills
  • Demonstrable experience of previous management of projects

Qualifications

Essential

  • Project management qualification or equivalent level of knowledge and experience
