An exciting opportunity has risen within the Urgent Care & Reablement service, for Band 2 Support Workers to join a community based multi-disciplinary team, providing enablement support and rehabilitation services for patient's presenting with an acute deterioration in health and/or functional ability.

The aim of the service is to prevent and reduce avoidable hospital admissions and facilitate early hospital discharges by supporting patients through a re-ablement process in order to maximise independence; identifying when this is not achievable and signposting accordingly to ensure appropriate long-term support.

Main duties of the job

As a valued team member your development will be supported through a robust induction programme, a value based appraisal, one to one meetings, regular in-service training and clinical and managerial supervision. You will work out of a multi-professional base so there is opportunity for joint working [including Nurses, Care Co-ordinator, Physiotherapists and Occupational Therapists, Support Workers and other associated health care professionals] and collaborative learning across a range of Health Professionals who are supporting and delivering high standards of clinical care.

Part time hours available:

Early: 08.00 - 14.00

Late: 16.00 - 22.00

Adult Physical Health

Coventry and Warwickshire Partnership NHS Trust (CWPT) has taken the decision to no longer continue to provide Adult Physical Community Services, as there are more suitable providers within the wider NHS local system.

Following a review of suitable providers to take forward the contract, the Coventry and Warwickshire Integrated Care Board (ICB) has confirmed its intention to award the contract for the provision of Adult Physical Community Services to University Hospitals Coventry & Warwickshire NHS Trust (UHCW). This change is anticipated to take effect from 1st July 2024.

Your application to work in Adult Physical Community Services will not be adversely impacted by the change in provider. Should you commence employment prior to the transfer, your contract of employment (that is your terms and conditions of service as at the point of transfer) will automatically transfer to the new organisation, under the "Transfer of Undertakings (Protection of Employment) Regulations 2006" (TUPE) as amended by the "Collective Redundancies and Transfer of Undertakings (Protection of Employment) (Amendment) Regulations 2014".

Should you commence employment after the transfer, your contract of employment will be issued by the new provider of Adult Physical Community Services.

To be an integral part of the Multi Disciplinary Team, providing planned programmes of support to enable daily living within the person's own home environment.

To promote effective communication with the individual and all other involved in their care, ensuring information is given as needed.

To report on progress with the individuals programme of support, in a timely and effective manner to the relevant professional

Use opportunities to promote health education, gaining advice from other professionals where necessary eg healthy balanced diet.
