The post holder will be working with specialist consultants, ward managers and nursing teams providing a clinical pharmacy service in supporting acutely unwell patients. You will ensure that safe and effective systems are in place for the use of medicines. You will help drive the medicines optimisation agenda forward through audit, research and innovation to ensure patients receive the very best pharmaceutical care tailored to their needs.

Main duties of the job
As a Senior Specialist Pharmacist, youll work across both our inpatients and outpatient areas, using your expertise to provide support and advice to our multi-disciplinary teams.You will also be able to grow your career as you will be exposed to advanced care pathways by leading Cardiac Consultants and other members of the MDT team. As a hospital based Specialist Pharmacist, you will work within our wards and dispensary to provide the best possible support and counselling to both inpatients and outpatients. In return you will become part of a network of peers who will support you with your learning and development through the opportunity to gain an Independent Prescribing Course

Job description
Job responsibilities
- The postholder will support the Lead Clinical Pharmacist in maintaining an excellent patient-centred service across Adult ITU and Cardiology at the Harley Street Clinic. This includes inpatient & outpatient areas.
- The postholder will deliver an efficient and effective, quality driven, pharmacy service to consistently meet the needs of all service users to the highest possible standard.
- The postholder will support the Head of Pharmacy with Medicines Management, Pharmacy Practice, Quality Assurance, Risk Management, and Clinical Governance matters by contributing to the development of activities and policies.
KEY TASKS & RESPONSIBILITIES:
Clinical
1. To provide a clinical pharmacy service to Adult ITU and Cardiology and to support other clinical areas such as surgery if required.
2. To demonstrate and apply specialist clinical pharmacy knowledge in order to plan, manage, monitor and review drug therapy for patients.
3. To provide a comprehensive pharmaceutical service to patients, staff and practitioners including prescription monitoring, provision of drug information, evidence-based practice and patient counselling.
4. To promote the active involvement of Pharmacy within the multidisciplinary team.
5. To participate on ward rounds, influence prescribers and recommend changes to patients treatment where appropriate.
6. To contribute to the development of policies, procedures, guidelines and setting standards aimed at improving the effectiveness, safety & efficiency of clinical pharmacy services and clinical services within The Harley Street Clinic.
7. Work with your line-manager to identify opportunities and risks, and to develop and improve the quality of service for the Pharmacy service.
8. To help with the monitoring and implementation of drug clinical guideline policies.
9. To undertake audits as required.
10. To contribute to quality improvement.
11. To establish links with other Adult ITU/Cardiac Pharmacists and promote good networking practices.
12. To undertake membership/representation within appropriate clinical forums (at local & national level)

13. To coordinate all aspects of pharmacy input to adult intensive care prescribing.
14. Following appropriate training, to participate in and lead Adult Parenteral Nutrition ward rounds with the dietician and place orders as required.
15. To be the pharmacy representative on The Harley Street Clinic Adult Critical Care team.

- What youll bring:
- - Registered with GPhC
- - Post graduate diploma/masters in clinical pharmacy or equivalent
- - Specialist clinical pharmacy knowledge and experience in specialties including cardiovascular & critical care
- - Extensive hospital clinical pharmacy practice experience, including some of above specialties
