Job summary

We are looking for a enthusiastic motivated individual to join our expanding 2 hour Urgent Response Team based in Oldham community. This is an exciting opportunity to be involved in the integrated health and social care organisation.

You should have excellent communication and interpersonal skills which will be utilized with in this multi-disciplinary health and social care teams; You must be able to provide clinical support to patients in their own homes.

You must have a keen eye for monitoring patent condition, and have the confidence to escalate patients appropriately. You will be closely supported by the team leads but will also be expected to work independently in the community.

Main duties of the job

You will be expected to work within the 2hr UCR team as part of the MDT providing full holistic care and assessments, to patient in need of urgent health or social care support working as part of the community integrated services in partnership with Enablement, NWAS, Age UK and social to name a few.

You will be expected to manage your patient caseload within your scope of practice, ensuring we meet the 2hr response target set of the team in line with the home first model. The ethos of the service is to support patients in their own home or usual place of residence avoiding unnecessary ambulance call out and A&E attendances.

We welcome forward thinking individuals who would like to be part of a dynamic urgent care response team covering the Oldham area supporting the home first model.Due to the nature of the post it is expected that you will have a full driving license and will have use of an appropriate vehicle for work purposes.

  • Registered Nursing Associate on the NMC Register

Knowledge, Training & Experience

Essential

  • Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the Organisation, and how the role may contribute to service development
  • Understands and acts in line with NMC professional standards for practice contained within The Code
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
  • Understand requirements for NMC professional revalidation
  • Understanding of the importance of following procedures and treatment plans

Desirable

  • Experience of working in a community, urgent care setting

Qualifications

Essential

  • Qualified Nursing Associate with Foundation Degree
  • GCSEs Grade C or above in Maths and English Language / New GCSE Grade 4-9 in Maths and English Language OR Functional Skills Level 2 in Maths and English or Equivalent

Skills & Abilities

Essential

  • Ability to organise and prioritise own delegated workload
  • Ability to deal with non-routine and unpredictable nature of workload and individual patient contact
  • Ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team
  • Ability to support, supervise, assess, and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting
  • Ability to travel to different geographical locations across the Trust footprint in line with service needs

Desirable

  • Full UK driving license with willingness to use own car for work purposes
