Main duties of the job

In accordance with the practice timetable, the post-holder will undertake a variety of duties which include but not limited to:

  • Conducting consultations with patients to assess their medical needs and provide appropriate treatment and advice.
  • Performing clinical examinations and diagnostic procedures as required.
  • Prescribing medications and monitoring patient response to treatment
  • Maintaining accurate and up-to-date medical records, utilising Ardens within SystmOne (full training provided)
  • Participating in multidisciplinary team meetings and case discussions.
  • Collaborating with other healthcare professionals to ensure coordinated and comprehensive patient care.
  • Participating in quality improvement initiatives and clinical audits to enhance patient outcomes and service delivery.

St Andrews Surgery is an inner-city practice located in the heart of a Hessle Road Hull. We are teaching training practice with two partners, a salaried GP, a strong nursing team and in house pharmacist who provide services to 8200 patients.

We are a young, dynamic, family friendly practice who believe in providing continuity for our patients whilst striving to provide the best possible care to the patient population.

We are part of the Symphonie primary care network and offer services such as extended access, social prescribers, health and wellbeing coaches and paramedic practitioners.

  • In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • Developing care plans for health in consultation with patients and in line with current practice disease management protocols
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Collecting data for audit purposes
  • Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
  • Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
  • In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Person Specification

Qualifications

Essential

  • Fully Qualified General Practitioner registered with the General Medical council (GMC)

Desirable

  • Long Term Condition training.
  • Evidence of professional development.
