We are looking for an enthusiastic individual to embark on a development journey and join our specialist community midwifery team for women with vulnerabilities - the Indigo Team. The post holder will work alongside our dynamic community midwifery teams, to be able to reach out to the women and families that are most in need and provide tailored antenatal and postnatal care to meet their needs. We believe that utilising specialist, trained maternity support workers will make a significant difference to the accessibility of services and maternity experiences and outcomes to vulnerable families.

The South Tees NHS Trust provides maternity services to women and families from across Middlesbrough and Redcar and Cleveland. These areas are some of the most deprived in the country with Middlesbrough ranking in the top 5 most deprived areas of the UK and 40% of its wards being in the top 3% most deprived nationally. Our communities experience many of the issues associated with deprivation such as high unemployment, poor population health, high levels of substance addictions, high rates of crime including domestic violence, high numbers of children in care and a significant number of children living in poverty. Both Middlesbrough and Redcar and Cleveland have seen an increase in the number of children living in poverty at well above the national average and Middlesbrough has the highest numbers of children living in poverty nationally.

Main duties of the job

In this role you will be responsible for;

  • Providing our most vulnerable families with tailored, specialist support to enhance their care
  • Working within the Community Midwifery Vulnerability Team (Indigo Team) providing personalised care plans around mental health, substance misuse and smoking, teenage pregnancy, infant feeding and domestic violence
  • Working as part of the multi-agency team around the family including through safeguarding procedures
  • Signposting to community and specialist services and working within the wider multidisciplinary agencies, initiating links within the Family Hub services
  • Focusing on the support of women with identified mental health needs and assessing emotional wellbeing
  • Promoting physical wellbeing, such as providing aqua natal classes and walking groups. Co-deliver Antenatal Programmes relevant to each local authority health visiting team
  • Act as 'gatekeepers' within the community, enabling Family Hubs to become 'one stop shops' for community services and signposting women to appropriate support
  • Linking with the Best Start peer support service and other commissioned and third sector organisations and organise appropriate transition of care and support
  • Delivering essential baby items and equipment sourced via local baby banks
  • Supporting the learning needs of student midwives
  • Data collection and evaluation of the role throughout its implementatio

Experience

Essential

  • Experience of working in health or social care.
  • Experience of undertaking and recording clinical observations and escalation of changes or findings.

Education & Qualifications

  • Numeracy and Literacy Level 2
  • Care certificate
  • Level 3 vocational qualification or Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties and/or Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3; -Theoretical knowledge -Experience of a range of care and related procedures -clinical observations -knowledge of relevant legislation
