Job Title: Community Psychiatric Nurse

Trust Location: Kent and Medway NHS & Social Care Partnership Trust

Pay Rates:£18 - £28 Per Hour

What youll be responsible for:

Providing robust assessment of patients (by telephone or face-to-face) who have been referred to the CMHT due to presenting with mental health difficulties.

Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.

Signposting to appropriate care providers (including primary and tertiary care). Attending daily Red Board meetings.

Raising concerns to senior staff for support and management. CompletingRiOdocumentation and planning care for patients.

Presenting cases to Multi-Disciplinary Team for discussion around care pathway.

Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.

The role will involve, but not be limited to you performing the above.

Qualifications

Essential

  • A minimum of 6 months paid experiences a Community Psychiatric Nurse in an UK hospital environment within the last 3years.
  • Comply with NMC Professional Registration and keep up to date with current nursing practices.
  • *Full UK driving licence and own vehicle.
