The Learning and disability Team has an opportunity for an experienced band 6 Social Worker who is keen to work with young people who are struggling with their mental health in both a community & inpatient setting.

You will provide a comprehensive, effective & integrated social work service to young people & their families & carers using our services.

You will work closely with & be an active and vital member of the MDT, supporting colleagues in relation to childrens social care, safeguarding needs & mental health legislative guidance.

Case Load: Likely 10 on caseload potential for more depending on complexity

Remote Working: Can be 75% remote apart from client visits and supervisions but also can be in office depending on candidate

Main duties of the job

Complete carers assessments and seek feedback from people who use the service.

Learning some basic clinical skills to enable the undertaking of holistic assessments and liaison with the multi-disciplinary team.

Supervise and/or train students and Band 5 colleagues as and when required to ensure their practice is safe and professional.

Provide supervision on a 1:1 basis or through peer supervision as and when required to ensure practice is safe and professional.

Ensure records are timely, kept up-to-date, accurate and used in accordance with the GDPR and organisational guidelines.

Design, implement and review care and support plans, which may involve the input of other professionals /disciplines.

Job responsibilities

The post holder is required to discharge statutory health and social care functions on behalf of the organisation and to uphold the standards outlined in the Professional Capabilities Framework for Social Work. As such, they are required to undertake complex and proportionate assessments that are evidence-based.

Examples include mental capacity assessments and health and social care assessments of people who have a variety of needs that may create a high level of risk to their wellbeing. The post holder is also required to arrange and review the delivery of social care support for health funded individuals.

Wherever possible the post holder is required to support people to take risks in complex situations so that their outcomes and wishes are promoted. The post holder is also required to consider decisions made in a persons best interest that may deprive them of their liberty and follow legal process to ensure legal frameworks are in place.

Person Specification

Qualifications

Essential

  • SWE (Social Work England) Registration
  • Degree or equivalent in Social Work e.g. DipSW, CQSW, CSS

Experience

Essential

  • Community Experience
  • Worked with Court of Protection
  • Experience with working in an MDT, (Multi-Disciplinary Team)
  • Worked with / full aware of the Framework for NHS Continuing Healthcare (CHC)

Desirable

  • Knowledge of S1(system 1) and Eclipse,
  • Should have one or the other knowledge. If you have Eclipse you can get started and be trained on S1.
