Hours: up to 37.5 hours per week. Core hours Monday to Friday
Salary: up to £34,233(depending on experience)

Responsible for: Working under the
direction of the Head of Hospice Dementia Team, with the support of senior
colleagues develop accountability for undertaking specialist holistic
assessment and management of dementia patients and their families/carers
Main duties of the job
An
exciting development post for a RN/SRN working towards becoming a DNS over the next year/18 months.
Working at an enhanced practice level with
support from senior colleagues to deliver a co-ordinated and comprehensive
specialist dementia service to patients and their families/carers.
This will include developing knowledge, skills
and experience with support from senior colleagues to undertake independent and
autonomous practice: comprehensive holistic assessments, agreeing specialist
palliative treatment management plans and advance care planning to be delivered
through community and out-patient settings.
The post holder will develop to become an
expert role model, supporting senior colleagues to deliver education and advice
regarding dementia care issues to other health and social care professionals.
Working with all members of the Hospice Dementia Team to stimulate and direct
the development of clinical practice across the local community.
There is a genuine occupational requirement for
the post holder to drive and have access to appropriate transport during the
course of normal duties.

Job responsibilities
Key Responsibilities
Regular attendance of the monthly Saturday
Dementia Café supporting attendees from the local community in addition to
known patients and families/carers
Provide
individualised care and prioritise needs in line with available resources and
considering local and national policy/guidelines to achieve the best possible
outcomes for patients and their families/carers.
Ensure that patients dignity, needs, wishes
and culture are always promoted and respected and that equality and diversity
are priorities of care.
Undertake comprehensive patient cantered
assessments, to enable patients and families to make informed decisions and
agree a plan of care based on clear goals intended to bring about effective,
measurable outcomes.
With guidance from senior colleagues refer to
primary and secondary care for relevant diagnostic investigations. Use a high
level of clinical enquiry to interpret results or to raise concerns.
With support from senior team colleagues/
medical team or GP review medication regimes and seek to optimize patient
concordance and reduce polypharmacy. Proactively oversee anticipatory
prescribing to ensure all necessary medication is in place to achieve
management of patient symptoms towards end of life in their preferred place of
death.
Work in partnership with other members of the
multidisciplinary team and services to help ensure patients achieve their
preferred place of care, utilising end of life tools such as the Gold Standard
Framework, Advance Care Plans and RESPECT documents.
Promote the work of the organization in all
settings, encouraging collaborative working and the referral of every appropriate
patient.

Qualifications
Essential
- Possess a Level 1 Nurse qualification with a minimum of 2 years post registration experience.
Experience
Essential
- Previous community/ dementia nursing experience
Desirable
- Have undertaken ENB 998 or other relevant teaching/mentorship training with the ability to deliver training programs with confidence and clarity.
