Job summary
We are looking for
a friendly, enthusiastic and motivated practice nurse to work alongside our
existing healthcare team.
Experience of
general practice is essential along with knowledge of supporting patients with
Long Term Conditions such as Diabetes and COPD. Other duties will be delivering
a wide range of services including cervical screening, immunisations for adults
and children and wound care.
Part - time Mondays and Wednesdays
Salary will depend
on experience
This is a permanent
post, ideally we are looking for someone already working as a practice nurse.
We reserve the

implementation of care to the individual patient
To follow protocols
and guidelines in place to maintain high standards of care.
Professional
Responsibilities and Duties
To be an effective
member of the Primary Health Care Team using knowledge and skills to provide
high standards of nursing care.
To maintain ones
own education allowing for good and up to date standard of care to be provided
to the patients.
To be able to
evaluate and implement individual care using practice protocols and guidelines.
To maintain
accurate nursing interventions on patients records on SystmOne computer system.
To ensure that
strict confidentiality is maintained.
To communicate with
other members of the Primary Health Care Team.
To acknowledge ones
own limitations of competencies as set out in the NMC Scope of Professional
Practice.
To attend and
participate in Nursing Team meetings.
To be aware and
proactive in Health and Safety issues in the working environment and maintain
high standards of Infection Control.
Educational
Responsibilities
To maintain and
develop ones own professional knowledge and competencies in accordance with the
NMC guidelines by:-
Keeping abreast of
current developments in Nursing.
Attending in
service multidisciplinary training and practice nurse training.
Take an active part
in staff development.
To deliver health
promotion issues so encouraging the patients to develop a healthier life style
by making informed choices.
