Job summary

Our innovative community pain / CFS/ ME pathway delivers an evidenced- based service tailored to the needs of patients with persistent pain. As a clinician working with the medicines pathway, you will need to demonstrate enhanced skills and knowledge of long-term pain and its impact upon individuals. This will include a sound knowledge base of NICE guidance and Local Area Prescribing Committee protocols as well as experience of polypharmacy. You will need to have excellent communication skills in this challenging clinical field and build links across the health economy anddeliver education sessions raising awareness of long-term pain management. You need to be an excellent team player, dynamic and forward thinking with ambition for self-development and strategic service development You will provide clinical leadership on medicines optimisation and quality improvement

Main duties of the job

KEY RESPONSIBILITIES

  • To provide a senior highly specialised clinical role in the Nottinghamshire Interdisciplinary Community Pain and ME/ CFS Pathway
  • To develop and contribute to the de-prescribing pathway and liaise with PCN/ Practice based colleagues in primary care
  • To undertake a medication review of complex patients in an outpatient setting or by telephone and formulate a management plan.
  • To plan and take responsibility for the mutually agreed patient care plan.
  • To have the relevant skills and qualifications to undertake medication reviews and initiate or amend pain medication regimes.

KEY REQUIREMENTS

  • Advanced Nurse Practitioner or Clinical Pharmacist with proven experience of working with patients with long term pain and associated conditions
  • In depth knowledge of analgesics and pharmacological management of long-term pain
  • Experience of conducting medication reviews either face to face or over the telephone

Job responsibilities

JOB PURPOSE

  • To provide a highly specialised clinical role in the Nottinghamshire Interdisciplinary Community Pain / ME/ CFS Pathway
  • To develop and contribute to the de-prescribing pathway and liaise with PCN/ Practice based Pharmacists in primary care
  • To undertake a medication review of complex patients in an outpatient setting or by telephone and formulate a management plan.
  • To decide on and take responsibility for the mutually agreed patient care plan.
  • To have the relevant skills to undertake medication reviews and initiate or amend pain medication regimes.
  • To work closely with pathway colleagues to develop a complex opioid clinic in Mid Notts
  • Maintain expert practice in all aspects of pain management, extending scope of practice and developing an advanced level of professional autonomy and accountability in the provision of such care
  • To demonstrate advanced teaching, sharing and innovating skills.
  • To take a lead role in facilitating best practise, service modernisation, clinical audit and research.

KEY RESPONSIBILITIES.

  • To undertake an advanced role that requires extending scope of practice and developing an advanced level of professional autonomy and accountability in the provision of such care.
  • To demonstrate clinical expertise, supporting the development of pain awareness and knowledge across the local health economy.
  • To liaise with primary and secondary care professionals regarding referrals to and from the service and be a clinical resource for advice on the management of persistent pain

Professional

  • To adhere to each professions code of conduct and practice at all times
  • To provide clinical education, training and evaluation of clinical competence of team staff providing support and guidance
  • To answer medication queries from other members of the team
  • To maintain, develop and disseminate specialist knowledge of evidence based practice.
  • To critically evaluate own work through the use of evidence based projects, audits and outcome measures.
  • To ensure that the professional development of the team meets national standards and that the team will have the appropriate skills to meet the needs of patients in a specialist clinical area
  • To lead on the promotion and development of best practice. Recommending and implementing changes to improve service delivery.
  • To continue to develop and implement evidence based approached for the management of persistent pain, as part of an inter-disciplinary team, with particular reference to core profession.

Communication and Relationship Skills

  • Ensure close liaison with Line Manager and Clinical Lead in communicating with regards to clinical issues. Organise and attend regular MDT / Supervision
  • Facilitates the communication of highly complex information regarding specialist issues on a range of service developments within a interdisciplinary team. This communication is directed to professional colleagues, across all areas of the health economy and all GP practices in the ICS
  • Advanced communication skills are necessary to communicate with patients to gain consent for treatment within a care pathway. Highly sensitive and contentious information is regularly required to be communicated to patients after clinical and medical results are collated formulating specific pain management plans.

Knowledge, Training and Experience

  • Highly developed specialist professional knowledge and skill beyond that associated with traditional -professional roles, including in-depth awareness of other health and allied health professionals skills.
  • Ensure that statutory legal requirements for medicines management are met in accordance with the legislation, local and national policies procedures and guidelines.

Analytical and Judgement Skills

  • During every day clinical practise, the interpretation of patient history and analysis of physical and biopsychosocial assessment, interpretation of medical investigations all require constant skills of analysis and judgement to expert level.
  • These skills are used to make clinical judgments in a context of absolute autonomy.
  • Continual professional development is concomitant with this post and has a professional responsibility for registration.

Planning and Organisational Skills

  • To plan and deliver training to staff across the health economy
  • To assist operational and clinical leaders in developing strategic plans as required.

Physical Skills

  • Be able to carry out assessments and treatments with moderate to intense effort on a daily basis.
  • To comply with the local Manual Handling Policy and local therapeutic handling guidance at all times.
  • Travelling between more than one site base will be necessary and therefore a reasonable level of fitness required. A car driver essential as travelling to multiple sites will be required

Responsibility for Patient Care

  • To act as an expert practitioner demonstrating advanced knowledge and skill beyond that associated with traditional professional roles, including the integration of research evidence into practice by expert clinical reasoning and decision making.
  • To be professionally and legally responsible and accountable for all aspects of the practitioners professional activities.
  • To create, develop and design with other multi-professional team members care pathways for referral of the particular groups of patients with the aim of disseminating good practice an improving outcomes throughout the trust.
  • To make independent clinical decisions and initiate investigations, care/treatment and technological intervention as required
  • To use expert theoretical and practical knowledge to refer to other departments/ colleagues within or outside PICS as necessary relying on own interpretation of assessment and expert clinical decision-making.
  • To work in collaboration with clinical teams/general management, in order to ensure there is a defined pathway of care for a patient with pain
  • To ensure that appropriate, consistent and realistic information is provided to patients and relatives.
