Job summary
An exciting opportunity has arisen for an enthusiastic Occupational Therapist
to join our palliative rehabilitation and wellbeing team.
You will be using your
specialist professional skills and knowledge to improve quality of life for those
diagnosed with a life limiting condition, as well as people accessing our
frailty services. You will empower and educate patients, community members and
unpaid carers, to better understand and self-manage their symptoms, to enable
patients to reconnect / stay connected to occupations that are important to
them.
You will be working as
part of our rehab and wellbeing team, predominantly within our Inpatient Unit,
but also supporting our community services, and carry out one to one
interventions and group interventions.
You must be a practicing
HCPC registered Occupational Therapist, with a minimum of 18 months
post-graduation experience in a variety of clinical areas.
Main duties of the job
The Band 6
Occupational Therapist will provide
Occupational Therapy assessment, treatment, and advice, which especially
focuses on wellbeing, self-management, independence, and confidence for
patients with a wide range of life limiting, palliative diagnosis.
The Occupational Therapist will work within the
multi-disciplinary team (MDT) to meet the needs of patients accessing Garden
House Hospice Care services, based within the day services (Rehab and
Wellbeing) team.
To utilise and promote a rehabilitative palliative
care approach to:
Provide Occupational Therapy interventions for patients attending Day
services, In Patient Unit and the community hubs.
Manage a caseload, using evidence based/patient centred principles
within physical and psychological medicine to maintain independence and promote
wellbeing.
Support patients and those important to them with a palliative life
limiting condition.
To maintain clinical records in accordance with legal requirements and
professional standards.

Qualifications
Essential
- Degree in Occupational Therapy
- HCPC Registration
- Minimum 18 months post qualification experience

- Evidence of continued professional development CPD portfolio
- Post graduation experience in a range of general clinical settings e.g. care of the elderly, neurological rehabilitation, respiratory, community intermediate care
- Thorough understanding and clinical reasoning skills of equipment provision
- Ability to manage a complex caseload in various clinical settings
- Ability to assess patients needs that may be complex and subject to change, and support discharge planning.
Desirable
- Post registration clinical experience specific to working in palliative care rehabilitation.
