Job summary

We have two exciting positions available for a Crisis Care Practitioner Nurse (Band 6) in the Hammersmith and Fulham Crisis Assessment and Home Treatment Team (CAHTT), West London NHS Trust.

We are looking for two new staff to join our vibrant and supportive MDT!

The post holder will be responsible for the assessment, co-ordination and the delivery of a high quality mental health service for patients under the care of CATT who require assessment by secondary mental health services for pathway planning, crisis intervention in the community, early discharge or gate keeping for the inpatient wards and community recovery house with the emphasis for potential providing home treatment.

The post holder will be involved in the on-going development of services, to ensure that the most relevant care pathway and care plan is devised to provide access to urgent and crisis care. They will be active in the promotion and use of evidence-based practice through the provision of clinical advice and support; acting as a role model; and providing education and training where appropriate. They will liaise closely with carers, both statutory and non-statutory services, emergency services, and third sector services. They will liaise closely with local services Single Point of Access providing rapid response to mental health crisis situations where necessary and appropriate to decrease the number of unnecessary presentations to A&E.

Main duties of the job

To carry out biopsychosocial assessments for individuals referred to the service in a mental health crisis.

To provide assessment for gatekeeping for inpatient admissions.

To provide support to junior colleagues and mentor student nurses.

To be part of an MDT and actively participate and be part of care planning and risk assessment/management.

To be able to administer and monitor medication in line with NMC code of conduct and Medicines management training.

To be conduct home visits working autonomy and as part of a team.

To work closely and liaise with other internal services and external agencies.

Be prepared to engage in professional development and training where appropriate.

Have excellent communication skills both verbal and written.

A high standard of clinical work to be carried out in all duties.

Ensuring compliance with clinical governance and quality improvement.

Qualifications

Essential

  • Registered Professional (Mental Health Nurse.
  • Relevant post registration experience.
  • Evidence of post registration further development.
  • Mentorship qualification

Desirable

  • Degree in Health Care related subject.
  • Qualification in psychosocial interventions in mental health.
  • KUF training

Experience

Essential

  • Post qualification experience to include appropriate experience within an acute mental health care setting and / or significant experience within community, crisis care, or liaison psychiatry.
  • Experience of working with other professions within a multi-disciplinary team.
  • Experience of working independently

Desirable

  • Experience of mentoring students.
  • Experience with telephone triage within a Single Point of Access Service.
  • Advanced Assessment Skills training

Knowledge/skills

Essential

  • A high level of both written and spoken English language
  • Knowledge of adult and children safeguarding requirements.
  • Knowledge of relevant mental health law.
  • Availability to work a shift rota covering 7 days a week 365 days a year
  • Full UK driving licence and willing to drive pool cars.
  • Ability to manage potentially distressing situations and events.
  • Punctual & Good attendance record.

Desirable

  • Knowledge of RiO patient record system.
  • Ability to communicate in a second language.
