Job summary

Brent Integrated Care Partnership has a number of exciting opportunities available, supporting the borough based partnership to deliver its objectives in a changing and evolving health and care landscape. We have 11 roles available, hosted by CNWL but working as part of a wider partnership of health and care organisations in Brent.

Main duties of the job

The Programme Manager will be responsible for the operational delivery and transformation of various community health services with a focus on, but not limited to, Care Home and Diabetes programmes.

In addition, the post holder will be supporting the Head of Care Pathways on Hospital Discharge initiatives and any other transformation priorities in Brent. The post holder will represent Brent at North West London discussions in order to implement sector wide transformation work streams locally.

The postholder will lead the development and implementation of the Community health services in Brent by working collaboratively with the Community Service Providers, Primary Care Networks, acute Trusts, Adult and Social Care team and the Voluntary Sector.

The post will liaise with the North West London Integrated Care Board (ICB) Team to ensure local implementation of sector wide work streams.

Because we want the roles to be available to people from a range of different professional backgrounds, these roles are being advertised in CNWL and at Brent Council. The roles will be based at Brent Civic Centre in Wembley, and will involve 3 days working in the office per week, with home working available for the other days.

MAIN DUTIES AND RESPONSIBILITIES

OPERATIONAL

  • To oversee team members to deliver the requirements listed above; engage and liaise with key stakeholder, in particular:
    • Manage team and drive delivery of a range of business initiatives and projects
    • Lead the development and implementation of the Community health services in Brent by working collaboratively with the Community Service Providers, Primary Care Networks, acute Trusts, Adult and Social Care team and the Voluntary Sector.
    • Liaise with the North West London Integrated Care Board (ICB) Team to ensure local implementation of sector wide work streams.
    • Support the identification and sharing of best practice through engagement
    • Operate in a highly political and sensitive environment
    • Support the portfolio of initiatives in demonstrating value for money for the current spend through tracking, managing and delivering agreed benefits
    • Be responsible for ensuring that procedures and policies are developed, reviewed and implemented in respect of document control, version control, variation process, project plan development, monitoring and reporting.

Person Specification

Education & Qualifications

Essential

  • Educated to a degree level or equivalent level of experience of working at a senior level.
  • Evidence of post qualifying and continuing professional development
  • Educated to master's level or equivalent level of experience or working at a senior level in specialist area

Desirable

  • Project management qualification, or equivalent experience in programme/project delivery roles

Experience

Essential

  • Experience in leading transformation projects within health and/or social care settings.
  • Must have an understanding of the background to and aims of current healthcare policy in London and appreciate the implications of this on engagement.
  • Have an appreciation of the relationship between NHS England and individual provider and commissioning organisations
  • Experience of managing and motivating a team and reviewing performance of the individuals

Desirable

  • Knowledge and experience in Training and Education or leading workforce related programmes.
  • Experience in stakeholder engagement and co-production
  • Comprehensive experience of project principles techniques and tools such as Prince 2 and Managing Successful Projects

Skills and knowledge

Essential

  • Developed communication skills for delivery of key messages to a range of stakeholders, both internal and external to the organisation.
  • Financial and staff management abilities, including budget management experience.

Desirable

  • Good IT skills across broad range of IT platforms.
