We are looking for a salaried ANP (prescribing) at our innovative GP practice now run by the GP federation, where we champion the doctor-patient relationship and continuity of care at practice level, underpinned by the expertise of the federation and at scale services. The Much Birch GMS contract was novated to the Federation in February 2024, but the practice team remains unchanged as does the identity of the surgery in the local community. Much Birch is :-

  • A busy rural dispensing practice at the heart of the community.
  • Located in the village of Much Birch, between Ross-on-Wye and Hereford city.
  • Covering an area reaching from the edges of Hereford to Ross-on-Wye and extending into the villages of the Golden Valley.
  • List size of circa 5000+ patients
  • A supportive clinical team of GPs, ANPs, Paramedics, Practice Nurses, HCAs, Physiotherapist, Social Prescriber & Remote Pharmacists.

Job Summary

  • The role will provide a specialist resource as an Advanced Nurse Practitioner for our patients working clinicians, nursing team and in collaboration with other members of the practice's multi-disciplinary teams.
  • To work within our practice community, as an autonomous, accountable advanced nurse practitioner providing holistic care whilst assessing, diagnosing and providing treatment to patients.
  • To use competent and up to date clinical skills.
  • To assess, diagnose, treat, refer or direct to other services; our patients who attend surgery or request domicile visits relating to unscheduled minor conditions, injuries, or acute illnesses.
  • To promote self-care and educate service users to allow them to be empowered to make informed choices about their treatment.
  • The post holder will have clinical supervision from a GP available at all times, and would be expected to liaise with said GP about clinical decisions.

Job Responsibilities

Clinical

  • To practice as an advanced nurse practitioner and demonstrate advanced clinical competence and knowledge about those associated with conventional nursing roles.
  • Assess, plan, treat and refer patients appropriately, carrying out consultations in the surgery or the community as an autonomous practitioner.
  • Using own clinical judgement to diagnose and treat patients either face-to-face consultation, clinical telephone triage or other mediums.
  • Making appropriate referrals to primary, secondary and social services care, following practice (and/or locally agreed) pathways and protocols.
  • Deliver high-quality clinical care in a timely manner to patients.
  • Supply, administer and prescribe medicines as indicated (and appropriate) to address patient need, according to medicines management policy, Patient Group Directions (PGDs), NMC standards or referring to a registered GP as necessary.
  • To recognise the need for and initiate where appropriate relevant diagnostic investigations, using their skills in this area to take the correct course of action.
  • Understanding local safeguarding referral process and be aware of appropriate referral mechanisms for children and vulnerable adults.
  • Ensure complete and accurate documentation of every patient contact, including advice given. Records must be accurate and reflect the clinical activity in accordance with NMC guidelines on clinical record keeping.
  • Minor illness assessment, diagnosis and treatment with access to seek advice, review and onward referral within clinical teams.
  • To develop care plans for patients when required (as developed and supervised with the Partners).
  • Be involved with continuity of care of our patients e.g., end of life care, chronic disease management.
  • Ensure the practitioner has the appropriate indemnity cover in place.

Clinical Governance

  • To participate in the clinical governance requirements within the practice.
  • To practice within the parameters of practice policies, protocols and guidelines.
  • To participate in quality improvement within the practice as necessary.
  • Reflect and assess results of own decisions, outcomes and treatment; identify any shortfalls in delivery of patient services and report to relevant management.
  • Ensure they follow health and safety legislation and COSHH regulations, reporting any breaches as necessary.
  • Awareness of personal responsibilities in maintaining knowledge of PGDs, medicines policy and legislation regarding safe and secure medicines management.
  • Take part in research and development activities where necessary (as directed by the Lead GP, Partners and/or senior management team).

Continued Professional Development & Learning

  • Maintain clinical knowledge and practice to standards of capabilities and competencies expected of the position of Advanced Clinical Practitioner.
  • To participate in the ongoing clinical supervision framework and individual professional development for clinicians within the Practice.
  • Ensure registration and qualifications with the Health and Care Professionals Council is kept up to date.
  • Be involved in ongoing education within the practice; to support and provide training to staff members as identified through clinical supervision sessions and reflective practice with clinical colleagues.
  • Actively participate in annual performance review (with necessary audits), engage in self-appraisal and provide clear objectives and evidence and develop continuous professional development plans.

Professional

  • Effectively manage own time, workload and resources

Administration

  • Participate in the administrative and professional responsibilities of the practice team.
  • Ensure appropriate expenses are made accurately and submitted to the Finance Manager.
  • Ensure all necessary paperwork and registration documentation is completed and submitted to the Quality Assurance Manager.
  • Postholder has a legal responsibility to comply with the Health and Safety at Work Act 1974, other relevant information and any risk or safety related issues within the Practice in the interests of staff, patients and contractors.
  • Aware of their responsibilities under the Health Act 2008 Code of Practice for the Prevention and control of Healthcare Associated Infections.

Health and Safety

The post-holder will implement and manage their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include (but will not be limited to):

  • Using personal security systems within the workplace according to practice guidelines
  • Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines
  • Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
  • Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering and availability
  • Work with the lead nurse on implementing hand hygiene across the practice
  • Work with the lead nurse on infection control and clinically based patient care protocols, and implementation of those protocols across the practice
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process
  • Making effective use of training to update knowledge and skills
  • Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes etc are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.
  • Safe management of sharps procedures including use, storage and disposal
  • Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management
  • Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised
  • Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers
  • Undertaking periodic infection control training (minimum annually)
  • Routine management of own team areas, and maintenance of work space standards
  • Waste management including collection, handling, segregation, container management, storage and collection
  • Spillage control procedures, management and training

It is the responsibility of each member of staff to ensure that all personal data relating to patients and members of staff, whether held in manual or electronic format, is always kept secure.

In the course of carrying out the duties as an advanced nurse practitioner , the postholder may have access to confidential information relating to patients and their carers, practice staff and others. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.

Any information relating to the above may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data. This work is of a confidential nature and information gained must not be communicated to other persons except in the recognised course of duty.

Person Specification

Qualifications

Essential

  • Professional registration with either NMC, GPhC, or HCPC
  • Evidence of MSc level education such as:
  • MSc Advanced Practice
  • Equivalent/other MSc mapped to the four pillars of advanced practice
  • Demonstrated competence at an advance practice level through Royal College or HEE credentialling.
  • Evidence of CPD
  • Non-medical prescribing if appropriate to registered profession/role.
  • Relevant role specific qualifications (i.e. Advanced Resuscitation, ALS/EPLS/ATLS, surgical skills qualification or radiology interpretation skills)
  • Leadership/management course or validated portfolio evidence (if not part of MSc level qualification)

Experience

Essential

  • Significant demonstrable experience of working as a registered practitioner in a provider organisation (e.g. acute sector, community provider, third sector, social care or primary care) and within a relevant specialty (suggested 4-5 years minimum)
  • Demonstrate an up to date and detailed knowledge of relevant guidelines and clinical practice in the specialty.
  • Evidence of recent teaching, mentorship, and supervision of staff.
  • Evidence of recent audit and/or research experience.

Desirable

  • Evidence of involvement in change management and/or clinical service improvement.
