Job summary

An exciting opportunity has arisen for an enthusiastic, conscientious, passionate and motivated occupational therapist with a passion for working with our community beds team. We are the clinical beds team with Nottingham city that provide assessment and rehabilitation to patients who are unable to be discharged home from hospital due to requiring a period of assessment or rehabilitation, or to patients who require a step up from the community to prevent a hospital admission.

The service consists of a multi-disciplinary team that includes Physiotherapists, Occupational Therapists, Nurses, Registered Mental Health Nurses, Assistant Practitioners, Rehabilitation Support Workers, and admin support. The aim of the MDT is to provide holistic assessments to patients within our clinical bed, promoting independence and arranging safe discharges either back to the patients own home or on to long term care.

You should have an understanding of Health and Social care provision and the ability to work closely and collaboratively with partner organisations and other stakeholders, such as social care and our community health partners.

You will be supported to hone your assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and reablement programmes, as well as complex discharge planning experience.

CityCare will offer you an excellent Preceptorship Programme to support & develop newly registered AHPs as well as a bespoke local induction programme.

Main duties of the job

Clinical Skills

  • To assess for, provide, demonstrate and fit adaptive equipment using problem solving, and analytical skills.
  • To accept clinical responsibility for a designated caseload of clients, and to organise this effectively with regard to clinical priorities and use of time and ability to be flexible and respond at short notice to the needs of the service.

Professional

  • To undertake the comprehensive assessment of clients, including those with a complex presentation using investigative and analytical skills through standardised and non-standardised assessments. Formulate individualised treatment plans with rehabilitation goals using clinical reasoning in consultation with senior therapists as required. This should be based on sound theoretical knowledge, utilising a wide range of treatment skills, and options to formulate a specialised programme of care.
  • To advocate on behalf of clients and carers, where appropriate, overcoming barriers to communication such as cultural or language difficulties, physical or mental health difficulties, using skills of empathy to facilitate or empower the client.
  • To be involved in the development of good practice for physiotherapists within Nottingham CityCare Partnership CIC and Social care under the guidance of senior staff.
  • To complete patient/client records in line with organisation, regulatory body and legal requirements.
  • To represent the service externally and actively participate in the services marketing strategy when required, conducting presentations to a wider audience, including voluntary and public sector agencies.
  • To demonstrate a commitment to evidence-based practice and the use of outcome measures and make recommendations for change. To have a good understanding of the research process, and be willing to participate in local research and audit when appropriate.
  • To be responsible for own Continuing Professional Development, keeping abreast of new recommendations, research and developments, incorporating them into your work as necessary. Attend relevant conferences and external training courses. Support more junior staff to do likewise.

Special Requirements

Essential

  • All appointments are subject to satisfactory medical clearance and the ability to cope with the demands of the role.
  • Ability to undertake frequent moderate physical effort for several short periods during the working day. Activities may include manoeuvring people, and operating equipment. Periods of prolonged concentration as work pattern dictates and some exposure to distressing or emotional circumstances and occasional unpleasant conditions.
  • Able to work unsocial hours and weekends within service hours of either 8:00-16:00, 12:00-20:00 Monday to Friday, 10:00-18:00 at the weekend.
  • Full driving licence, use of vehicle for work with business insurance.

Experience

Essential

  • Relevant undergraduate experience working with adults including Health Care of the Elderly and Older peoples mental health / Integrated Medicine, Orthopaedics, Neurology and Musculoskeletal.
  • Understanding of multi-disciplinary and/or multi-agency working.
  • Competence in individual caseload management and ability to manage time / work under pressure/in terms of risk and priorities.
  • Customer service skills.

Desirable

  • Post graduate experience working with adults including orthopaedics, neurology, falls prevention and musculoskeletal.
  • Full KSF for the role/other relevant competency framework.
  • Community experience. Lone working.
  • Experience of working within multicultural communities.
  • Understanding of Social Enterprise.

Qualifications

Essential

  • BSc/Diploma in Occupational Therapy.
  • HCPC Registration.
