Central London Community Health are excited to announce the opportunity to employ an enthusiastic, compassionate and proactive General Practitioner with Interest in health Inequalities Brent Health Matters - Children and Young People , within the expanding team. This new, exciting, and challenging role offers the opportunity to join a newly established team including two senior nurse clinicians, a healthcare coordinator and a service lead, in the ongoing development of the Brent Health Matters Flagship Model for health inequalities in the community.

This is an exciting role for someone with an interest in health inequalities and able to understand the current barriers affecting our population group along with utilising your existing knowledge of Brent and experience of working in Brent with Brent GPs.

Main duties of the job

In this role, you will be required to liaise with the registered child's GP for families who seldomly access health care, provide professional support to the services, clinically review and advise on abnormal investigations and medical escalations. You may occasionally support community events that are offered as part of the wider Brent Health Matters programme delivery.

We are currently recruiting to 1 session/ week for the children's BHM team - the main scope of the role will be to work alongside the clinical lead to ensure appropriate clinical governance across the service, to lead/ participate in a multidisciplinary team meeting, to support the team with clinical queries and to support liaison back with GP practices. If you have the passion to continue our ongoing work to support our identified patient cohorts and we particularly welcome applications from a diverse range of backgrounds or at any stage in their career.

You will need to be flexible in your approach to work, the role may include travel across the Borough

Experience in community services would be beneficial.

The desirable candidate will have a knowledge of working in Brent and with Brent GPs.

Job description

Job responsibilities

The post holder will proactively work collaboratively with service partners to identify and engage children and young people who are not engaging with health and care services.

To lead a weekly Multidisciplinary team meeting to discuss the BHM caseload of Children and Young People not engaging with usual health and social care services.

To facilitate and support patient centred care for hard-to-serve children and young people living in the community to ensure high quality experience and outcomes.

To provide strategic input into efforts to increase uptake of children and young people immunisations and health screening.

To provide specific case review of the patient that may require medical input.

To be responsible for clinical governance of the service and have an oversight of clinical pathways and standard operating procedures used by the team and review of quality KPIs.

To provide clinical supervision/mentoring for the team.

To assist with GP and PCN liaisons including feedback and strategy from Team visits and case findings.

Education/Qualification

Essential

  • Full Registration with the GMC
  • MRCGP Registration
  • General Practice experience
  • Paediatric Life support or Higher
  • Regular updating for Child Safeguarding and Adult Safeguarding
  • Satisfactory Current Appraisal

Experience

Essential

  • Post registration Primary Care Experience
  • Relevant Clinical experience with Families, Children and Young People
  • Experience and Knowledge within Health Inequalities and Long-term condition management
  • Partnership working with multi agency/system partners to deliver proactive effective seamless services in the community.
