Job summary

As part of a multidisciplinary team, you will offer specialist recovery support to clients receiving support for detoxification from a number of substances. You will be an experienced and passionate about supporting clients achieve personal recovery goals.

As an integral part of the service, you will be responsible for coordinatinga caseload of up to 18 residents with the support of volunteers. Your day to day work could include delivering groupwork sessions, offering 1-2-1 key work appointments, liaising with community recovery organisations and other professionals as well as encouraging and reassuring new admissions. You will be a vital link between a clients community support both prior to admission and upon discharge.

Job description

Job responsibilities

Work in a small but fast paced detoxification unit, offering 24 hour care to complex clients.

In partnership with clinical colleagues, assess, plan and evaluate care for patients, taking into account the potential needs and care plans with additional regard to safeguarding, physical/mental health, Blood Bourne Virus, sexual health, smoking cessation.

Continuation and initiation of care planning a clients recovery journey, which is person centered and puts the client at the focus of all decision making.

Exploring Compliance to treatment and where necessary testing.

Delivery of a wide range of evidence based and holistic interventions, both on a 1-2-1 basis and in groups.

Taking a lead role in specific areas of interest, offering added value to The Pavilion offer.

Joint multidisciplinary collaboration with all stakeholders.

Caseload of complex and non-complex clients.

Delivery of KPIs in line with contract agreement to help evidence our service delivery.

To support the service to maintain at least a Good rating from the Care Quality Commission and strive for Outstanding.

Supporting colleagues and managers with new initiatives and service offers, and any reasonable requested Activity.

Person Specification

Qualifications

Essential

  • -Health and Social Care NVQ Level 3 or equivalent
  • -1 years experience working with clients experiencing drug and alcohol issues

Experience

  • -Ability to deliver group work and 1-2-1 sessions
  • -A proactive and flexible approach to service delivery
  • -Experience of supporting volunteers
  • -The ability to work evenings and weekends on a rota basis
  • -A proactive approach to supporting clients to achieve their personal recovery goals
