Job summary

We are looking for a highly motivated, enthusiastic part-time (2 days a week) Practice Nurse to join our complement of nursing team. We currently have 3 Practice Nurses, and an HCA working alongside 5 GP partners and 3 salaried GPs.

Lawton House Surgery is a friendly and forward-thinking practice serving 12,575 patients in East Cheshire. The surgery was completely extended and refurbished in 2019 allowing us to look at expanding our services.

The role is varied and covers all aspects of practice nursing including Chronic Disease Management (experience in diabetes would be of particular advantage), cytology, Adult and childhood immunisations, contraception and general treatment room duties.

Provide nursing treatments to patients in participation with the general practitioners or independently to agreed protocols.

While Practice nurse experience is desirable, consideration would be given to the right candidate able to demonstrate an aptitude to make the move into General practice, or a recently qualified practice nurse who can demonstrate an aptitude to advance and thrive within a Primary care setting.

Main duties of the job

  • Experience of working within accordance with the NMC code of professional conduct.

  • Experience of working with the public/patients

  • Effective communication with interpersonal skills.

  • Knowledge and understanding of current national developments in healthcare eg NICE guidelines.

  • Experience of managing patients with Asthma/COPD and working within General Practice

  • Computer literate - the Practice uses EMIS clinical system

Salary - negotiable dependent on experience

Hours up to 15 hours are available between the hours 7:00am - 8:00pm.

Initially on a Monday - 10:30am - 7:00pm & Tuesday 8:30am - 5:00pm.These working hours will be subject to change.

As our Primary Care Network develops there will be an opportunity to increase hours, possibly working in the other Primary Care Network practices.

Job responsibilities

Job Summary:

To assist medical personnel in the care of Practice patients to include treatment, preventative care, screening and patient education.

Job Responsibilities:

Providing assessment, screening and treatment services and health education advice to provide chronic disease management clinics for patients.

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.

Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to general practitioners as necessary.

Advise patients on general health care and minor ailments with referral to GPs as necessary.

Supplies and equipment Treatment room and other areas:

In participation with the Practice Manager, support the HCAs to ensure the maintenance of equipment and stock levels relating to patient care.

Pathological specimens and investigatory procedures:

Following appropriate training, undertake the collection, storage and despatch of pathological specimens including blood samples, swabs, smears etc. and ensure the cleanliness of any equipment so used. Performa any investigatory procedures requested by the GPs.

Administrative and professional responsibilities:

Participate in the administrative and professional responsibilities of the Practice Team.

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Attend and participate in Practice Clinical meetings as required.

Research projects:

Co-operate and participate as required in any research projects within the Practice.

Person Specification

Experience

Essential

  • Experience of working within a GP Medical practice.
  • A demonstrable commitment to professional development.

Desirable

  • Experience of working in primary care in a GP practice.

Qualifications

Essential

  • Registered General Nurse
  • Recognised qualification to diploma level or above

Desirable

  • Experience in Chronic Disease Management ( diabetes would be an advantage) and respiratory conditions . Adult and childhood immunisations, contraception and general treatment room duties.
  • Experience of working within General Practice
