Job summary

We are seeking an experienced nurse to join our Community Paediatric service with a focus on assessment and support for young people with, or suspected of having, a neurodevelopmental condition including autism, ADHD or a learning disability. As well as being involved in the delivery of care, you will have a leadership role within the team supported by senior nursing colleagues.

Our Community Paediatric service is made up of a multi-disciplinary team including Paediatricians, Nurses, Speech & Language Therapists, Occupational Therapists, Physiotherapists, Support Workers & Associate Practitioners, Clinical Psychologists and Wellbeing Practitioners. We work closely with colleagues from partner agencies across Plymouth.

We welcome applications from all branches of nursing, but you must have demonstrable experience working with young people who have a learning disability or neurodevelopmental condition.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

  • To support the senior nursing team for Community Paediatrics in the ongoing operational management of the Nursing Services within the Child Development Centre.
  • In conjunction with the Lead Nurse for Community Paediatrics ensure the continued provision of high quality care to all children through staff education, governance and professional development.
  • To act an as an expert in neurodevelopmental conditions, co-ordinating care pathways and developing the service to meet national guidelines and standards.
  • To co-ordinate the multi-professional approach in the provision care for children who have a learning disability, neurodevelopmental condition or related needs
  • Work collaboratively with colleagues across University Hospitals Plymouth and partner organisations in developing services and delivering assessment & support for young people & their families.

Job responsibilities

  1. Ensuring the efficient and effective provision of care, advice, education and support to children and families in their own home or other community settings.
  2. Assist the Lead Nurse for Community Paediatrics with future service development by identifying service need, evaluating priorities, service analysis.
  3. Undertake project management as agreed with Lead Nurse for Community Paediatrics and the Directorate manager
  4. Contribute to the co-ordination, collation and analysis of health data regarding children and young people within the CDC.
  5. Responsibility for the line management of staff working with young people who have a neurodevelopmental condition.
  6. Implement and facilitate the appraisal system in line with the knowledge and skills framework
  7. Initiate and participate in the recruitment process within HR and recruitment guidelines
  8. Promote and ensure the provision and delivery of the highest quality of care for children and their families including the establishment of quality standards and implementation of quality initiatives.
  9. Maintain an active role in all aspects of clinical governance including clinical audit, management of complaints, incidents and risk.
  10. Ensure that the training need analysis are carried out and implemented as necessary and that all mandatory training is kept up to date.
  11. To problem solve, and respond appropriately to both personal and professional matters as they occur.
  12. As an autonomous practitioner manage a caseload of children with continuing medical/nursing care needs, ensure that through the ongoing assessment process their continuing needs are met.
  13. Prioritise and undertake visits to children and their families at home or in other community settings to deliver appropriate care / intervention.
  14. Lead on the nursing assessment of children attending the assessment groups.
  15. Undertake Nurse led clinics for children attending the Child Development Centre
  16. Support or coordinate the multi-disciplinary assessment of young people identified as having a possible neurodevelopmental condition
  17. Using evidence based practice ensure that the children and their families receive the appropriate individualised care as necessary
  18. Liaise where required with the learning disability and autism liaison services within University Hospitals Plymouth NHS Trust.
  19. As part of the multi-disciplinary team assist in the implementation of care packages in the community. Undertaking nursing assessments as requested as part of the continuing Health Care process.
  20. Act as a mentor to both junior staff and students ensuring that their training and learning needs are met.
  21. Lead on clinical survey, research and audit programmes as indicated.
  22. Undertake the role of Child Protection (CP) Supervisor, ensuring regular individual and group CP meetings providing advice and support to staff in relation to their direct contact with families. Liaise with the Trust Named Nurse for Safeguarding as appropriate.
  23. To develop and maintain an informed awareness of safeguarding children to protect the welfare of children.
  24. To work in close collaboration with practitioners and service managers to ensure effective working practice across all agencies.
  25. Initiate, lead and co-ordinate common assessment framework meetings for children in need of multi-disciplinary agency and safeguarding support providing reports for and attending conferences and courts as requested.

Person Specification

Knowledge and experience

Essential

  • Proven post registration demonstrable experience in relevant speciality.
  • Able to demonstrate specialist proven knowledge in relation to ADHD assessment and management.
  • Current and up to date demonstrable experience of nursing young people with neurodevelopmental conditions / learning disabilities.
  • Able to organise team workloads effectively
  • Current proven knowledge of multi-disciplinary / multi-agency roles and responsibilities
  • Significant proven knowledge and demonstrable experience of safeguarding procedures
  • Demonstrable experience of undertaking audits
  • Relevant demonstrable experience of Clinical Governance

  • Degree level qualification in relevant field
