As a Case Leader you will be the focal point of contact and guidance
to our Support Workers who work closely with our clients on a regular
basis.
You will require a UK full driving license and vehicle due to the role
being community based.

The successful candidate will also need to be willing to complete clinical training in accordance with the needs of our clients.
Main duties of the job
As a Case Leader you will be supervising an allocated caseload of clients and
staff teams.
You will engage closely with our clients, family members and other
healthcare professionals involved in their care.
You will act as a role model
to our staff team and monitor, mentor and develop our staff team.
One of
your main aims in your role will be inspiring our Support Workers to be
focussed on achieving their clients specific goals and outcomes.
You will
need to be organised and able to plan your own diary and ensure your visibility
and involvement of each assigned clients support package.
You will need to be
able to communicate effectively with clients, families, staff and other
professionals involved with each of your clients.
The role of Case Leader with Caremark involves conducting daily, weekly and
monthly reviews with our Support Workers such as:

Case Leader
Job Description
Case Leader
will report to the Registered Care Manager Diane Taylor
Our Case Leaders will supervise an allocated caseload of
clients and staff teams. They will engage closely with our clients, family
members and professionals and they will monitor, support and develop a
dedicated staff team, motivating and inspiring them to be focussed on achieving
their clients goals and outcomes.
Our Complex Case Leader role is community based, and as such,
it is important that the successful individual is organised, has strong
planning skills and can operate on their own as well as being part of our well
established team. We will expect the successful individual to be highly visible
in the community, visiting their assigned clients on a regular basis and being
a pivotal part of the service that we provide.
It is important that the successful individual will be
flexible as and when required to ensure that the service is monitored and
supported as needed. For example, some of our clients have night support, or
weekend support, and as such, regular spot checks and observations will need to
be completed at these times. In addition to this, some of our clients have
support at school and will need engagement through the day. Therefore, the
successful person will need to manage their own diary in order to meet the requirements
of the service whilst also maintaining a good work-life balance.
Our Case Leader will participate in an on-call rota, which
is the services emergency line out of hours.

Responsibilities
Our Case Leaders are expected to:
Monitor and audit our service on a daily basis,
ensuring high standards of care are delivered to each client
Engage with and support our staff team through Team
Meetings, Supervisions, Spot Checks, Annual Reviews and Observations
Conduct regular reviews with clients, involving family
members, professionals and advocates where necessary
Monitor, audit and maintain records within the clients
homes, ensure log books/eMAR charts and monitoring charts are collected and
re-delivered on a monthly basis
Introduce new staff members to their new clients,
ensure staff are fully aware of the clients individual care and support
agreement and are focussed on helping the client to achieve their goals,
outcomes and aspirations
Recognise compliments given by clients and family
members, and feed this information back to staff members through letters,
cards, team meetings and supervisions, to ensure they can be recognised
Monitor and maintain medication records in the
clients home and conduct live audits to ensure best medication practise.
Identify situations which may de-stabilise care
packages, and work in a pro-active way to identify solutions to any issues.
To liaise with other members of our management team,
including our Recruitment Officer, Care Co-ordinator and Compliance Officer to
identify solutions to staff levels, compliance and training records.
