Job summary

Practice nurses work in GP surgeries alongside other healthcare professionals including doctors, health visitors, pharmacists and dietitians. They provide treatment and advice to a wide range of patients, as well as administering tests to aid diagnosis.

Main duties of the job

3. Has specialist skills and expertise in one or more of the following underpinned by appropriate education:

Nurse led Minor Illness/Injury Clinic

Diabetes Management

Cardiovascular Disease Management

Respiratory Disease Management

Minor Illness

Triage (to include telephone assessments)

Travel Health Advice

Family Planning and Womens health

Sexual Health

4. Effectively and appropriately communicates with the multi-disciplinary team regarding the care of individual patients.

5. Accurately communicating results to patients where appropriate.

Administration

1. Maintain accurate clinical records; both electronic health records and paper records where appropriate.

2. Ensure treatment rooms are prepared for use, and equipment is kept in working order.

Procurement

1. Ensure stock rotation system is adhered to

2. Proactively orders replacement stock/consumables as per practice policies and procedures.

Bridge Street Medical Centre is an NHS GP surgery based in historic premises in the centre of Cambridge - our building was formerly home to the oldest scientific society in Cambridge, the Cambridge Philosophical Society.

We offer a variety of appointments to suit your needs and are usually able to offer you the clinician of your choice. We run on a named doctor basis- you will have an allocated GP who received all of your results and paperwork as well as ordinarily being your first point of contact - with the intention of offering the best possible continuity of care.Our surgery is open all day Monday to Friday from 08.00 to 18.00

Bridge Street Medical Centreiscommitted toteaching and training. As well as trainingthe GPs of the future, we often have student doctors from Cambridge and London universities at the surgery. You will always have the option to opt-out of being seen by students, but we value your support in helping them to develop clinical excellence.

The GP Surgery supports research and works closely with the Primary Care Research Network.If you areinvited to participate in research your rights as a patient will never be affected if you choose to refuse.

Job responsibilities

Primary Responsibilities:

  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
  • Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Provide vaccination and travel medicine services
  • Provide wound care (ulcer /Doppler etc.) to patients
  • Request pathology services as necessary
  • Process pathology results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure read codes are used effectively
  • Maintain chronic disease registers
  • Develop, implement, and embed well woman clinics
  • Develop, implement, and embed well man clinics
  • Implement vaccination programmes for adults and children
  • Prioritise health issues and intervene appropriately
  • Recognise, assess, and refer patients presenting with mental health needs
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies
  • Deliver opportunistic health promotion where appropriate

Secondary Responsibilities:

  • Develop, implement, and embed health promotion and well-being programmes
  • Implement and evaluate individual treatment plans for chronic disease patients
  • Assist GPs with minor surgery when required
  • Chaperone patients where necessary
  • Support the team in dealing with clinical emergencies
  • Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  • Act as the audit lead, effectively utilising the audit cycle
  • Monitor and ensure the safe storage, rotation, and disposal of medicines
  • Support junior members of the nursing team, providing guidance when necessary
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the practice
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • Participate in the review of significant and near-m

Person Specification

Qualifications

Essential

  • A new General Practice Nurse will be:
  • Registered on Part 1 of the NMC Register.
  • Have a degree or relevant previous experience.
