Job summary

An exciting opportunity exists for a Clinically Qualified and experienced nurse to work as a Tissue Viability Nurse (TVN) in Ealing Borough. The role provides an excellent opportunity for the successful candidate to utilise their skills and experiences in this specialist field as well as development opportunities.

The Team delivers a range of services, including;

Initial Specialist assessments from a range of referrers, including District Nurses, GPs and Community and acute hospitals as well as Nursing Homes

Utilising a range of resources, including Digital technologies

Advice and Guidance to District Nursing and other members of the MDT

Teaching and development of other staff and carers

Working both from Team bases and providing home visits as appropriate

Close partnership working with a range of other services and providers

In response you will work as part of a Specialist team of nurses and Health care professionals. You will be provided with Specialist Supervision and Guidance as well as opportunities to further develop your skills in this field

Main duties of the job

Establishing and maintaining relationships with Key stakeholders in the management of TV issues across Ealing Borough

Providing support to the wider MDT to deliver responsive, patient centred care in line with NICE guidance as well as local and National standards

Being a positive role model and setting standards for the team

Ensuring that the KPIs are delivered and any risks are escalated at the earliest opportunity to ensure that mitigation is in place.

Ensure compliance with our clinical governance framework, you will motivate colleagues, champion service user safety and dignity and drive continuous improvement.

Qualifications and Training

Essential

  • Registered Adult Nurse
  • Post registration qualification in wound care, tissue viability or leg ulcer management
  • Teaching qualification
  • Evidence of continued professional development

Experience

Essential

  • Experience in teaching theory and practical skills.

Desirable

  • Advanced nursing skills in the assessment, planning and evaluation of patients tissue viability needs.
  • Experience of working as a clinical nurse specialist.
  • Knowledge and experience of audit, research and quality improvement.
  • Knowledge and experience in clinical governance

Knowledge

Essential

  • Demonstrate knowledge of specialist pressure reducing equipment.
  • Demonstrate an in-depth knowledge of the intrinsic and extrinsic factors that affect wound healing.
  • Demonstrate knowledge to assess and diagnose wound aetiology.
  • Demonstrate knowledge of long term conditions and how they impact on wound healing

Desirable

  • Demonstrate knowledge of government frameworks and targets
