Llantrisant Dialysis Unit is a dynamic 21 station unit providing haemodialysis treatment for 84 patients in partnership with the NHS Trust. The unit operates safe staffing with a protected 3:1 patient to nurse ratio.

You will be fully supported and provided with excellent training and development which will enable you to provide safe, high quality, holistic nursing care to patients undergoing dialysis treatment. Main duties of the job

To develop and expand theoretical knowledge and practical skills involved in the care of a dialysis patient

To demonstrate competent and efficient patient assessment care needs, coordinating with the unit link role resources and multidisciplinary team, to ensure patients receive their optimum care

To demonstrate a proactive mentorship approach to the team, acting as a resource in the unit team, empowering patients, qualified nurses, ancillary staff and student nurses to share knowledge and skills.

Hours of work:

Full time role working 37.5 hours per week

3 x 11.5 hour shifts per week plus an additional top up shift every month

Long Day = 06:30 -18:00

Monday to Saturday on a rotad shift system

No Sunday working

No night shifts

We expect staff to work flexibly and work shifts depending on the need of the business

Job responsibilities

As an Avitum employee, you will demonstrate a commitment to abide by the Avitum UK Commitments and Guiding Principles and strive to perform according to these standards at all times.

To develop and expand theoretical knowledge and practical skills involved in the care of a dialysis patient:-

- to complete the Avitum induction programme within 3 days of employment start date.

- to undertake the Avitum staff development phased programme. This will be successfully completed within an agreed time frame with the Dialysis Unit Manager.

- to complete the Avitum annual mandatory training requirements.

- to attend Avitum training and study days as scheduled.

- to achieve annual performance management objectives and gateway transitions.

- optional enrolment in the access, renal & management courses.

To demonstrate an active and effective clinical practice, providing a safe, high quality care for patients, achieving compliance with established procedures and codes of conduct and practice at all times, recognising and accepting individual skill and knowledge limitations and reporting when medical intervention is necessary: -

- to demonstrate commitment to self-development, updating staff development training file.

- to provide evidence of post registration education practice as per Nursing & Midwifery Council requirements to maintain an active registration.

- to liaise with Dialysis Unit Manager, unit trainer, mentor and Training & Development Manager to establish appropriate annual performance management objectives.

To demonstrate competent and efficient patient assessment care needs, coordinating with the unit link role resources and multidisciplinary team, to ensure patients receive their optimum care :-

- to undertake a competent and accurate clinical pre & post dialysis assessment.

- to demonstrate a competent and accurate development of patient care plans.

- to demonstrate a competent and accurate implementation and evaluation of patient care.

- to demonstrate a competent and accurate monthly review of patient clinical data, providing evidence of urea kinetic monitoring and evaluation care to optimise clinical standard achievement.

- to demonstrate a competent and accurate monthly review of the patients dialysis prescription, providing evidence of evaluation care to optimise clinical standard achievement.

- to demonstrate a competent and accurate review of the patients coagulation management, with activated clotting time monitoring, providing evidence of evaluation care to optimise clinical standard achievement.

- to demonstrate a competent and accurate review of the patients anaemia management, providing evidence of evaluation care to optimise clinical standard achievement.

- to demonstrate a competent and accurate review of the patients bone disease profile, providing evidence of evaluation care to optimise clinical standard achievement.

- to demonstrate a competent and accurate infection control review of the patient and patients access, providing evidence of evaluation care to optimise clinical standard achievement.

- to demonstrate accurate and compliant patient assessment documentation.

To demonstrate accurate and competent evidence of the holistic dialysis care provided as a named nurse. To maintain an in-depth knowledge of the patients physical, psychological and social state, promoting the patients interests as a nurse advocate :-

- to maintain an awareness of all developments in the patients clinical timeline e.g. outpatient appointments, referrals, investigations, multidisciplinary team etc.

- to interact with the patient in the planning of their care to optimise empowerment and independence.

- to perform documented holistic assessments of the patient at least every 3 months.

- to identify problems and potential problems in conjunction with the patient, providing support in developing plans to address any issues.

- to competently select the appropriate care pathway for identified problems.

- to monitor access management 3 monthly and review results.

- to monitor activated clotting time ranges 3 monthly and review anticoagulation.

- to seek to interact with the patients family in their care, as per patient consent.

- to maintain the patients documentation in a compliant and legible order.

- to manage the monitoring of the patients Hepatitis and HIV status as per policy.

- to complete, competently and accurately, the patient manual handling assessments as per policy.

To demonstrate a proactive mentorship approach to the team, acting as a resource in the unit team, empowering patients, qualified nurses, ancillary staff and student nurses to share knowledge and skills.

To demonstrate a proactive motivation, competence and efficiency in the management of the unit on a daily basis, as per development performance management objectives, to ensure consistency, compliance and organisation are optimised on each shift:-

- to demonstrate a commitment to take charge of a unit shift as agreed with Dialysis Unit Manager.

- to demonstrate a commitment to acquiring the knowledge & skills required to achieve this, coordinating with your mentor and Dialysis Unit Manager.

- to demonstrate initiative in shift coordination in the absence of the Senior Dialysis Nurse.

- To develop initiative and skill in the compilation of the unit rota schedule.

To promote and maintain professional relationships with both patients and Trust staff to enhance the business excellence and partnership with B Braun Avitum UK Ltd.

To work within the Avitum Policies and Procedures and adhere to the Quality Manual and Environmental, Health and Safety Guidelines.
