A vacancy has arisen within the Unplanned Care Team, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges.

You will be leading a team of qualified nurses, nursing associates and health care support workers, supported by a community team leader, to take an innovative solution-focused approach to challenging situations.

Main duties of the job

The District Nurse Caseload Manager is a predominantly Clinical role, time will be split with an 80% Clinical and 20% Non-clinical focus. The Caseload Manager will be responsible for the day-to-day smooth running of the caseload and hold clinical responsibility for the caseload they are managing and leading on, ensuring patients are appropriately assessed and care is delivered in a timely manner with the involvement of patients and carers as appropriate.

The caseload holder will also ensure that team members are appropriately trained, supported and, have the right knowledge, skills, and competencies to meet the requirements of the role.

We are looking for a highly motivated applicant who is interested in meeting the challenge of working in the community with patients in their home setting. As part of the Unplanned care team, you will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients. You'll be working on a rotational basis as part of a seven-day service covering 24-hours-a-day with the support of agile technology to help you deliver timely, high-quality care.

  • Current NMC Registration/relevant allied health care professional qualification

  • Teaching/ Assessing in clinical practice

  • Qualification in the management of one or more chronic disease/LTC
