We have a full time vacancy for band 6 registered nurses .

End of Life Care within Walsall is rated as OUTSTANDING by CQC within community and the Hospice. The post holder will work within a multidisciplinary team who are focused on delivering the best holistic care possible to patients with a life limiting illness.

This post is part of an Integrated Specialist Palliative Team comprising of Consultants in Palliative Medicine, CNS' in Specialist Palliative Care in Hospital & Community and Therapies. This forms part of a wider Specialist Palliative Care group consisting of Community Palliative Therapies, Complementary Therapies, Day Hospice, Information & Support Services, Goscote Hospice, Bereavement & Lymphoedema.

Enthusiasm, motivation and compassion are qualities and skills considered essential for this role as you will be working in a challenging environment; you will be expected to have a commitment to and progressive work within the field of palliative and end of life care.

Main duties of the job

37 .5 hours per week - days, nights , weekends and bank holidays

  • Caring for patients with a life limiting illness likely to be in the last 12 months of life
  • Support symptom control management for patients attending with physical, psychological , emotional symptoms
  • Care for patients in the last days/weeks of life
  • Be part of wider Specialist Palliative Care MDT
  • Deliver fundamentals of care based on patient wishes
  • Support communication with carers
  • Link with teams across acute , community & primary care

Detailed job description and main responsibilities

Undertakes the provision of nursing care with support from senior colleagues. Will develop the skills and competencies to perform additional skills as related to the specialism. Will assist in the supervision of junior nursing staff and, where applicable, the teaching of students from differing disciplines eg nurses, AHP's paramedics. Will be required to participate in the implementation of audit and quality initiatives. Acts in a manner that upholds the values of the Trust and that of the profession.

Job responsibilities

  • Provision of specialist advice and education to the multi-professional teams within the hospital.
  • Undertake specialist assessments of patients with moderate palliative care needs and work with primary care givers/key workers to improve the quality of life of patients relatives and carers living with a life threatening and/or terminal illness; providing education for patients, relatives and carers.
  • Communicate effectively with patients, relatives and carers using specialist skills e.g. advanced communication skills as necessary.
  • To attend and actively contribute to regular palliative care and site specific multi-disciplinary team meeting (MDT)
  • To participate in the extended hours service for specialist palliative care.
  • Act as a specialist resource for information on all aspects of palliative & end of life care within Walsall Healthcare NHS Trust.

  • Post registration experience with Demonstrable experience within Palliative Care/ Oncology/Long Term Conditions
