Job summary

We are looking for an enthusiastic Practice Nurse to join our expanding team of nurses at our friendly practice for 13.5 hours per week (Tuesday 14:00-18:30, Wednesday 08:00-13:00 and Thursday 14:00-18:30)

Main duties of the job

Working as part of the practice multidisciplinary team, the post holder will ensure skilled nursing services are delivered effectively to the practice patient population. The practice nurse will be responsible for delivering health promotion, chronic disease management, health prevention, and well women clinics, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure

About us

We are GP teaching practice with a dedicated team committed to treat minor ailments, as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues for Brackley and the surrounding area. We have 6 partners, 5 salaried/locum GPs in addition to 15 staff in the Nursing Team and Urgent Care Unit for a 16000 patient population.

We are a SystmOne practice

In 2020 we moved to a purpose built location with excellent staff facilities and free parking.

We would be keen to support and develop areas of special interest within primary care

Job responsibilities

MAIN DUTIES:

  1. To assess patients health needs and implement nursing interventions to meet their needs.
  2. To manage the care and treatment of patients with stable or long term conditions within agreed protocols.
  3. To maintain high standards of infection control at all times, protecting patients, self and colleagues.
  4. To actively engage patients and carers in the management of their health, promoting health and wellbeing.
  5. To refer patients to other members of the practice clinical team in line with agreed procedures.
  6. To maintain accurate records of all clinical activity in line with practice protocols and NMC standards.
  7. To assist in specialist clinics within practice protocols.
  8. To contribute to clinical governance activities within the team including audit and evaluation.
  9. To support other members of the practice team in maintaining health, safety and security, identifying and assessing potential risks and contributing to management.
  10. To limit your actions to those which you feel competent to undertake. If you have any doubts about your competence during the course of your duties you must immediately speak to your line manager.
  11. To develop the role by using evidence-based practice and continuously improve self-knowledge following continuing professional development guidelines.

12. To use appraisal and supervision to identify development needs and maintain and develop competence in line with NMC requirements.

CLINICAL DUTIES:

Health Promotion

1. To recognise patients readiness to change in supporting their attempts to change unhealthy behaviour.

2. To provide healthy eating advice individually in opportunistic consultations.

3.To provide individual advice to patients about increasing activity or exercise.

4. To offer advice on recommended safe limits for alcohol consumption.

5. To assess, plan, provide and evaluate nursing care to meet the care needs of individuals and groups in the practice population.

Emergency Management

1. To be competent in anaphylaxis and resuscitation techniques.

Child Health/Protection

1. To recognise the signs, symptoms and categories of child abuse.

2. To understand the importance of recognition, observation, documentation and communication channels.

3. To be aware of statutory child health procedures and statutory local guidance and basic legal issues.

4. To be aware of other practitioners and agencies roles.

Family Planning

1. To advise patients on available methods of contraception and refer patients as appropriate.

2. To perform interval health checks for patients using hormonal contraception.

3. To advise patients on reducing the risk of sexually transmitted diseases.

Womens Health

1. To advise patients on the national and local cervical cytology and mammography screening programmes.

2. To advise on breast self-awareness.

3. To undertake cervical cytology sampling.

4. To take swabs for microbiology.

5. To give patients advice on the increased peri and post-menopausal risk of osteoporosis

Vaccinations and Immunisations

1. To administer injections under an individualised prescription or Patient Group Directive (PGD).

2. To ensure safe storage, rotation and disposal of vaccines.

3. To provide an adult and childhood immunisation service.

4. To maintain a call and recall system for patients.

5. To work to PGD for prescription only medicines.

Infection control

1. To apply infection control measures within the practice including:

i. Universal hygiene precautions

ii. Collection and handling of laboratory specimens

iii. Segregation and disposal of waste materials

iv. Decontamination of instruments and clinical equipment

v. Reporting on and treatment of sharps injuries

vi. Dealing with blood and bodily fluid spillages

Wound Management

1. To perform wound care using aseptic techniques.

2. To remove sutures and clips as clinically required.

Patient Screening

To undertake opportunistic screening, including blood pressure checks, height/weight and BMI calculations, urinalysis and smoking status

Person Specification

Experience

Essential

  • IT skills

Desirable

  • Experience of working in a primary care environment
  • Experience of chronic disease management
  • Immunization and vaccine experience
  • Willing to take appropriate training

Qualifications

Essential

  • Qualified Registered Nurse

Desirable

  • Post graduate diploma or degree (Chronic Disease Management)
  • Diploma in Asthma/COPD/diabetes
