Job summary

The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required.

Main duties of the job

Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition.

Proactively identify and manage treatment plans for patients at risk of developing a long-term condition as appropriate.

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.

Work with patients in order to support adherence to prescribed treatments.

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care.

Support and manage health needs of women presenting for family planning and cervical cytology consultations.

Recognise and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.

Implement and participate in vaccination and immunisation programmes for both adults and children.

Advise, support and administer vaccinations for patients travelling abroad.

Meet the needs of patients presenting for opportunistic wound care.

TheTrafalgar Medical Group Practice is a 27,000 patient list,6GP Partner Practice situated in the heart of Southsea. The Practice operates across 2 sites. The Practice is also the lead Practice for the Portsmouth South Coast Primary Care Network.

This is an established training Practice and we are looking for an experienced, enthusiastic and motivated Practice Nurse who can integrate well and who has skills to offer which can benefit our patients.

Previous experience within Primary Care would be desirable, but we will also consider applications from nurses whose background will also provide suitable experience and qualifications.

Hours are negotiable, but there will be a requirement to cover staff absences when required.

Salary - Competitive, subject to qualifications and experience.

Qualifications

Essential

  • Registered General Nurse.
  • A demonstrable commitment to professional development.
  • Experience of working in accordance with the UKCC Code of Professional Conduct and The Scope of Professional Practice.
  • Basic computer/typing skills.
  • Experience of dealing with the public / patients.
  • Competent in basic nursing duties required for the post.
  • Specific nursing skills / qualifications.
  • Excellent communication skills.
  • Previous experience within Primary Care would be desirable, but we will also consider applications from nurses whose background will also provide suitable experience and qualifications.
