Job summary

The SinglePoint of Access service is a newly developed team within the Hospice. This is an exciting time to join the team as you will be fundamental in supporting the ongoing development and growth of the service. The service will be responsible for receiving all external referrals into the hospice and establishing the need and urgency of the referrals, ensuring that the patient receives the right services at the right time. The team works in partnership with the wider clinical services to ensure patient flow and responsiveness. The post holder will work closely with the lead clinical assessor to allocate and contact patients and referrers to establish priority and provide a timely and efficient response to all referrals and record outcomes. They will also respond to advice calls in a timely manner and offer quality, professional advice and support to patients, families and health care professionals, and be able to signpost them as appropriate to other relevant services.

Main duties of the job

The main responsibilities include: -To assess and establish need, determine priority of referrals, and ensure referrers/patient/clients/carers receive a timely and efficient response based on presenting need. -To provide advice and support to referrers/patients/clients. -To work with the lead clinical assessor in the development of the SinglePoint assessment, process and policy. -To support in the continuous development of the SinglePoint of Access service through a quality improvement approach and contribute to the review and reporting of the service as required. -To work closely alongside patient/client facing teams to establish the flow of care and provide a timely response to referrals into services. -To work closely with and develop meaningful working relationships with primary care colleagues and other referrers to ensure that the processes of referral to the hospice are efficient, understood and feedback is provided. -To provide cross cover working with the lead clinical assessor for annual leave. -To maintain and promote effective and efficient working and communication with other community professionals, hospital professionals, voluntary, private and statutory agencies and other hospices. The post holder will have excellent communication and documentation skills and be a motivated and flexible worker. The post holder will work closely with the current SinglePoint team and wider team members within the organisation, to continue to develop the SinglePoint of Access service.

Person Specification

Qualifications

Essential

  • -Registered nurse or AHP
  • -Educated to Degree level or equivalent
  • -Evidence of continuing professional development

Desirable

  • -Post graduate qualification in palliative and end of life care
  • -Advanced communication skills training.
  • -Teach and mentoring qualification.

Experience

Essential

  • -Knowledge of palliative care and end of life symptom management.
  • -Ability to recognise and respond to potential distress in individuals and to manage crisis situations.
  • -Knowledge of Advance Care Planning
  • -Sensitive communication skills with patients with a life limiting illness and their carers.
  • -Experience of clinical audit
  • -Ability to recognise and escalate safeguarding concerns.

Desirable

  • -Experience of working in a palliative care setting or a setting which meets the needs of those with palliative care needs

  • -Experience of working with people with palliative and end of life care and support needs.
  • -Experience of working across and liaising with multidisciplinary and multi-agency teams.
  • -Excellent facilitation and negotiation skills.
