Job summary

Under the supervision of the Single Point of Access Team Manager, to act as the first point of contact for all clinical and patient referrals and enquiries which come in via telephone and email and process these using the Trust's EMIS system to appropriate clinicians across Community Services.

To act as a first point of contact for patients and Health Care Professionals through Single Point of Access and provide administrative support to multidisciplinary staff, whilst acting as a focal point for prioritising communication to provide seamless care to patients.

This role is for part time hours of 22.5 hours per week. The successful candidate will work on a shift basis starting between 8am and 12 noon and finishing between 4pm and 8pm (working 7.5 hours per day). The service operates 7 days a week so there will be a requirement to work a weekend day on a team rota basis. It would not be every weekend.

Main duties of the job

  • To act as a first point of contact for the Single Point of Access, record on the Trust's EMIS system and other databases patient details, reason for call and next appropriate action
  • Provide first point of contact for all communications, to sort, distribute and prioritise referrals from partners and outside agencies incoming by telephone and email using judgement and experience to complete the referral process to community teams
  • Ensure that patients' notes are treated confidentially and are correctly maintained following Caldicott guidelines and that documentation contained therein is correct, of a high quality of standard and is maintained at all times
  • Have sufficient knowledge of community services to ensure the efficient running of the service in relation to referrals and the timely and appropriate dissemination of information across the teams
  • Work flexibly across the locality where and when required
  • Perform ad hoc duties as and when requested by managers. To be aware that we are all working in a changing environment and therefore to be prepared to change working practices, should this be required, to promote a more efficient use of resources or better patient care

  • Keep the work place in a clean and tidy state
  • Develop skills so you can effectively communicate with patients of a great age range who may be hard of hearing, demented, confused or have difficulty with English. This will also include dealing with patient family members who may be calling in difficult circumstances

Qualifications

Essential

  • Evidence of continued professional development
  • GCSE /Equivalent maths and English (pass C or above) o
